[Objective]To analyze the risk factors for acute lung injury(ALI)early after orthotopic liver transplantation.[Methods]The perioperative clinical data of all 132 patients who had undergone orthotopic liver transplantation were analysed retrospectively. According to whether or not ALI developed posttransplantation,132 patients were divided into ALI group and NON-ALI group.Several statistically significant risk factors were screened out with univarite analysis,then independent risk factors were determined with multivariate stepwise logistic regression analysis.[Results]Of all the 132 patients,the morbidity of ALI was 12.12%with a mortality of 6.25%.Univariate analysis showed that the occurrence of ALI was associated with age,preoperative level of bilirubin, preoperative class of Child-pugh,urine volume during operation,the volume of blood transfusion during the operation,the volume of fluid infusion during the operation,the volume of blood loss during the operation,postoperative time of tracheal intubation,postoperative level of urea,postoperative Ccr,the equation of the liquid volume early 3 days after the operation,the fluid infusion volume early 3 days after the operation,the volume of blood transfusion early 3 days after the operation,and re-operation.Logistic regression revealed that re-operation,age,postoperative level of urea,urine volume during operation,the fluid infusion volume early 3 days after the operation are the risk factors for ALI.[Conclusion]The occurrence of acute lung injury is related to age,preoperative liver function,the volume of fluid infusion during and early after operation,the volume of blood transfusion,postoperation renal function and re-operation.
