Objective TO discuss appearance of bone marrow in children initial acute leukemia(AL) on routin MRI sequence,diffusion characteristic and the value of MRI in diagnosing AL.Methods MRI scanning of lumbar column(sagittal) were performed in study group(15 initial AL children manifested by marrow puncturing,including ALL 10 cases and AML 5 cases) and control group(12 cases).Signal intensity difference of bone marrow on FSE T2WI and FS-FSE T2WI were compared between the two groups.ON DWI,signal attenuation ratios(SARs) and apparent diffusion coefficients(ADCs) of leukemia and normal bone marrow were analyzed.Results(1)Routine MRI appearance:Signal intensity of all lumbar column bone marrow in children initial AL are hypointense on SE T1WI,isointence on FSE T2WI,hyperintense on FS-FSE T2WI.Signal intensity difference of bone marrow on FSE T2WI and FS-FSE T2WI of leukemia group(7.775±2.225) was lower than control group(111.995±27.989)(t value was 14.43,p<0.05).(2)SAR:On DWI,the SARs of leukemia and normal vertebrae were:b=400s/mm2-(33.51±6.64)%,(27.84±6.15)%;b=600s/mm2-(54.73±12.02)%,(37.15±6.20)%.The SARs of leukemia vertebtae were evidently higher than those of normal vertebrae at the same imaging parameters(t values were 4.57,2.26,p<0.05).In the same vertebrae,he higher b values,the higher SARs(t values were 5.98,3.70,p<0.05).(3)ADC:On DWI,the ADCs of leukemia vertebrae and normal vertebrae were: b=400s/mm2-1.17±0.26×10-3mm2/s,0.59±0.45×10-3mm2/s; b=600s/mm2-0.76±0.15×10-3mm2/s,0.48±0.02×10-3mm2/s.The ADCs of leukemia vertebrae were evidently higher than that of normal vertebrae(t values were 6.31和7.397,p<0.05 ).In the same vertebrae,the higher b values,the small ADCs(t values were5.161,7.978,p<0.05).Conelusion MRI appearance of lumbar column bone marrow of AL is diffusion abnormal signal intensity.DWI is a quantifictive and objective method in analying leukemia bone marrow.There is no different diffusion characteristic between ALL and AML lumbar column bone marrow.
