Objective:to study the vertical transmission rate of hepatitis B virus from father to infant.Methods:ⅠEnzyme linked immunosorbent assay(ELISA) and florescence quantitative polymerase chain reaction(FQ-PCR)1.131 pregnant women with both HBsAg and HBeAg negative were selected as the group of late pregnancy.71 cases of them,whose spouses were HBV carriers,were served as experimental group and other 60 cases,whose husband testing both HBsAg and HBeAg negative were served as control group.The serological marker of HBV and HBV-DNA were tested using ELISA and FQ-PCR respectively in the venous blood of the couples,the umbilical blood and the neonatal venous blood.2.8 pregnant women of induced labor(due to fetal abnormity or family planning) with both HBsAg and HBeAg negative were chosen as the group of induced labor group.3 cases of them,whose spouses were the carriers of HBV,were served as experimental group and 5 cases of them,whose husband testing both HBsAg and HBeAg negative were served as control group.The serological marker of HBV and HBV-DNA were detected using ELISA and FQ-PCR respectively in the venous blood of the couples,the umbilical blood or the heart blood of the fetus.ⅡPV-9000 immunohistochemistry1.Examined the expression of HBsAg on the placenta of experimental group of the group late pregnancy.2.Evaluated the expression of HBsAg on the placenta and the fetal liver of the induced labor group.3.9 pregnant women of induced abortion(due to family planning) with both HBsAg and HBeAg negative were selected as the group of early pregnancy.4 women of them, whose spouses were the carriers of HBV,were taken as experimental group and 5 couples of them,whose husband testing both HBsAg and HBeAg negative were served as control group.The expression of HBsAg was examined in the villus using PV-9000.Results:1.The positive rates of HBsAg in the umbilical blood and the neonatal venous blood of experimental group of the group of late pregnancy were both 0,the positive rates of HBV-DNA were also 0,the positive rates of HBsAb were both 85.9%(61/71),and there were no significant difference between them.2.The positive rates of HBsAg in the umbilical blood of experimental group and control group were both 0,the positive rates of HBV-DNA were 0 too,and there were no significant difference between them.3.The positive rates of HBsAb in pregnant women of experimental group and control group were respectively 90.1%(64/71)and 86.7%(52/60),there were no significant difference between them(p>0.05);When the HBsAb is positive in pregnant women, the positive rates of HBsAb in the umbilical blood of experimental group and control group were respectively 85.9%(61/71) and 76.7%(46/60),and there were no significant difference between them(p>0.05).4.The positive rates of HBcAb in the venous blood of pregnant women and the umbilical blood of experimental group were respectively 16.9%(12/71) and 29.6% (21/71),there were no significant difference between them(p>0.05).5.The immunohistochemical results of the placentas showed no yellowish brown color pellet expression in experimental group,but the positive control group showed yellowish brown color pellet expression and it was located in the cytoplasm.6.The hepatitis B serological results of the umbilical blood or heart blood of the induced labor showed HBsAg negative and HBV-DNA<1000 in all cases;The immunohistochemical results of the fetal livers and the placentas showed no yellowish brown color expression in experimental group and control group.7.The immunohistochemical results of the early pregnancy group showed no yellowish brown color pellet located in the cytoplasm in experimentaI group and control group.Conclusion:1.Based on the data found in the research,in the condition of inoculating the HBVac widespread in china,the positive rate of HBsAb is so high that the vertical transmission rate of HBV from father to fetus is very low.2.The umbilical blood could be used to screen of the hepatitis B transmission from father to infants after the fetus deliveried.
