Popliteal Artery Occlusive Disease: Etiology And Treatment

Abstract/Summary:
Objective:To investigate the etiological and surgical procedrues for Popliteal artery occlusive disease.Methods:From Jun.2007 to Jun.2008,25 patients with popliteal artery occlusive disease were analyzed retrospectively.Including 18 males and 7 females; average age,52.8±21.3 years old;11 patients(11 lower extreities,42.3%) with Acute limb ischemia and 14 patients(15 lower extremities 57.7%) with chronic limb ischemia. Although atherosclerosis is the most common cause of popliteal artery occlusion(50%), other conditions include:popliteal artery entrapment symdrom(23.1%),peripheral emboli(19.2%) and acute thrombosis from hypercoagulable states(7.7%).All patients were treated with surgical revascularization.Four patients underwent endarterectomy and angioplasty with patch above knee.Ten patients underwent thrombectomy, endarterectomy and angioplasty with patch below knee.Four patients underwent throbectomy only,Four patients(5 lower extremities) underwent resection of the aberrant muscle and angioplasty the popliteal artery with a patch.One patient underwent resection of the aberrant muscle and femoral-popliteal bypass with reversed saphenous vein.One patient underwent endarterectomy and femoral-popliteal bypass with graft. Results:Postoperative ankle-branch index(ABI) was significently higher than preoperative ABI,(0.75±0.29 vs 0.35±0.20,P<0.01).Average follow up time was 10.2 months.Two patients were amputated postoperatively.The rate of limb salvage was 92.3%in this study.Conclusion:The causes of popliteal artery occlusive disease were more diverse and complicated than the other lower extremity artery occlusive disease. Treatment for popliteal artery occlusive disease shall be consided about its etiology,and taken appropreately.The outcomes will be satisfied with an adviasble strategies.
