Objection: To provide evidence to elucidate the mechanism of tissue expansion by means of comparing the epidermal thickness and the percentage of p53-positive epidermal cell of normal skin and expanded skin. To enlarge the clinical use of tissue expansion.Methods: We draw specimens from 3 parts of a same patient: the most expanded skin(A), border expanded skin(B), normal skin(B). There were 10 patients and 30 specimens. Then we compared the epidermal thickness in the 3 parts of skin: A, B, C from HE stained slices, and compare the percentages of p53-positive cells in the 3 parts of skin: A, B, C from p53 immunohistochemistry stained slices.Result: The epidermis of A is thicker than that of B in 7 patients, butthere is no statistical significance. The epidermis of A is thicker than that of C in 9 patients, and there were 7 differences were statistically significant. The epidermis of B is thicker than C in 8 patients, and there were 6 differences were statistically significant between the two groups. The p53-positive cells of A are more than those of B in percentage in 7 patients, and there were 5 differences were statisticallysignificant between the two groups. The p53-positive cells of A are more than that of C in percentage in all 10 patients, and there were 8 differences were statistically significantbetween the two groups. The p53-positive cells of B are more than that of C in percentage in 9 patients, and there were 5 differences were statistically significant between the twogroups.Conclusion: The epidermis of skin proliferates due to the stimulation of expansion, and the epidermis may be thicker as the tension of expansion is higher. There was a higher expression level of p53 gene in expanded epidermis than normal epidermis. The possible reason is hypoxia, which can induce a high level of p53 expression. P53 protein may be a regulatory factor of tissue expansion.
