Objective to explore the role which the imflammation may play in the mechanism of diabetes and its cardiovascular complication,and the different levels of monocyte chemoattractant protein-1(MCP-1) are determined in the diabetic patients with and without atherosclerosis,the impaired glucose torlerance patients with and without atherosclerosis.Meanwhile the intimal media thickness(IMT) of bilateral carotid arteries is detected,the levels of MCP-1 and IMT are compared to find out the possible relation between the them.As a result,it may provide a theoretical support in preventing and healing the diabetes and its cardiovascular complication.Methods Patients with diabetes and patents with IGT and healthy persons as control group were screened from a population of 1231 residents at jiangnan community in guangzhou city.The ultrasonic inspection of Carotid artery were detected and they were divided into control group,IGT&AS,IGT,DM&AS,DM in all patients according to their intimal media thickness(IMT).The levels of MCP-1 were detected by Elisa,the different levels of MCP-1 were compared among these five groups.Meanwhile the levels of Total cholesterol(TC),Triglyceride(TG),low density lipoprotein-cholesterol(LDL-L),high density lipoprotein-cholester ol(HDL-L),systolic blood pressure(SBP),body mass index(BMI) are detectd,and the difference of the levels of the indicates above were compared among the five groups.The correlations between MCP-1 and IMT as well as other indicates above were researched.finally,using IMT with 1.2mm cutpoint as a dependent binary variable,BMI,HDL-C,LDL-C,TG,TC,FPG,SBP,MCP-1,age and gender were chosen as independent variables to make a binary logistic regression analysis.All results were delt with spss10.0 program.Result There was a significant difference in the levels of MCP-1 among the normal control group with(12.00±9.33)pg/mL,IGT group with(20.47±18.23)pg/mL and DM group with(36.33±13.14)pg/mL,P=0.000.the level of MCP-1 is significantly higher in IGT&AS group than in IGT group[(32.1±8.2) pg/mLvs(15.7±8.7)pg/mL,P=0.007],the level of MCP-1 was significantly higher in DM&AS group than in DM group[(43.5±8.8)pg/mL vs(28.3±12.8)pg/mL, P=0.014].The level of MCP-1 was significantly positively correlated with IMT (r=0.207,P=0.015),FPG(r=0.059,P=0.010),TG(r=0.198,P=0.029) and insignificantly correlated with SBP(r=0.101,P=0.220),TC(r=0.167,P=0.052),HDL-L(r=0.093,P=0.306),LDL-L(r=0.107,P=0.238),AGE(r=0.108,P=0.188),BMI(r=0.119,P=0.151),using IMT as a dependent variable in a forward LR Logistic regression analysis,gender,MCP-1 and age were independently correlated with IMT.CONCLUSION Monocyte chemoattractant protein-1(MCP-1) play an important role in the initiation and progression of insulin resistance,diabetes and cardiovascular comlication.MCP-1 may become a new predictor in cardiovascular complication of diabetes.
