Background and Objective: There are conflicting ideas about the therapeutic effect of varicoletomy(VT) at present, most scholars thinks that varicoletomy(VT) could improve the spermatic quality and fertility rate, but others consider that no subsequent drug treatment after varicoletomy may be the main reason of infertility still. Through clinical test, De Rose AF found that compare with varicoletomy merely, taking some antophysin after varicoletomy may be better in improving the spermatic quality related parameters as well as fertility rate, and the internal study had demonstrated it too. FR.Zhang had ever reported that ShaoFuZhuYu soup could inhibit the formation of granulation tumor and show anti-inflammatory effect in rats, and the rationale of ShaoFuZhuYu soup treatment to congestion is basic for it. WangXiaoLiu provided the evidence that giving ShaoFuZhuYu soup together with hormone treatment after varicoletomy shew a significant function in curing infertility. HuanYuanZhuYu(HYZY) soup was a amendment which is basic for ShaoFuZhuYu soup. Take HYZY soup to clinical application, we found that it could improve the clinical symptom, through making 20 cases of control studies about varicocele. The objective of this study is to investigate the therapeutic effect and adverse reaction of the ShaoFuZhuYu soup, and assess the influence of the spermatogenesis in the VC rat, provide a reliably experimental proof to supporting herbal therapeusis after varicoletomy in varicocele.Materials and methods:12 ones of 72 male SD rats(8w) were selected to be a sham operation group, the leaves were dealt by step1: created the varicocele model by partial ligation of the left renal vein, randomly divided them into 5 groups whereafter: the model, the VT, the VT with HYZY soup administrated in low, mid and high dose group, each of which contain 12 ones. In addition, in the sham operation group, the left renal vein was exposed without interference with it. Step2: manage the 4 groups-VT, VT with HYZY soup treatment in low, mid and high dose group by taking varicoletomy, except for the sham operation group and the model group. Herbal administration: the rats in the sham operation group were administrated Sodium Chloride by 1ml/100g wt. through intragastric administration per day; the model group: Sodium Chloride given by 1ml/100g wt. through intragastric administration per day; The VT group: Sodium Chloride given by 1ml/100g wt. through intragastric administration per day; the VT+HYZY soup in low dose group: the herbal was given by 0.8g/100g wt. through intragastric administration per day; the VT+HYZY soup in mid dose group: the herbal was given by 1.6g/100g wt. through intragastric administration per day; the VT+HYZY soup in high dose group: the herbal was given by 4.0g/100g wt. through intragastric administration per day. Every group was given the herbal for 60 days after step1 completed 30days later. When the Herbal-giving was over, we detected the dates such as 1.fertility rate, 2.left testis weight, 3.spermatic quality related parameters(sperm density, survival and motility), 4.detect apoptosis of apermatogenic cell by TUNEL, 5.observe testis ultrastructure by electron microscopy.Result:1. Fertility rate The fertility rate of the model group was obviously lower than that in sham operation group and VT+HYZY soup in mid as well as high dose group(P<0.05), but compared with VT group and VT+HYZY soup in low dose group, it shew no significant difference. In the other hand, compare VT group with VT+HYZY soup in mid and high groups, there are significant difference between them(P<0.05).2. Left testis weight The left testis weight of the model group was obviously lower than that in other groups(P<0.01); compare VT group with VT+HYZY soup in low, mid and high groups, there are significant difference among them(P<0.01). giving HYZY soup after varicoletomy could raise the left testis weight better than varicoletomy done merely.3. Spermatic quality related parameters3.1 sperm density Compared with other groups, the sperm density of model group was the lowest(P<0.01/P<0.05); there was no significant difference between VT group and VT+HYZY soup in low dose group in sperm density(P>0.05), but compared with VT+HYZY soup in mid and high dose group, VT group was much lower(P<0.01).3.2 sperm survival and motility There was a significant difference in sperm survival and motility between model group and other groups(P<0.01), it shew the worst spermatic quality in model group. Compared VT group with VT+HYZY soup in low dose group, there was no significant difference(P>0.05), however, with VT+HYZY soup in mid and high dose, it manifested a significant difference(P<0.01).4. Apoptosis of apermatogenic cell There was significant difference in AI of spermatogenic cell between model group and other ones(P<0.01), and compare VT group with VT+HYZY soup in low, mid and high dose group, there also shew a significant difference(P<0.01).5. Testis ultrastructure by electron microscopy Chromosome condensation and decomposition were seen in model group. In each group, we could see that the decomposition of the membranous composition of the apoptotic spermatogenic cells occurred, nuclear membrane became smooth, the round apoptotic bodies transmitted toward cellular membrane. But this change was comparatively slight in VT+HYZY soup in mid and high dose group.Conclusion: Experimental varicocele could do harm to the spermatogenetic cell in rats, increase incidence of apoptosis in spermatogenic cells. Management with varicoletomy could improve it in some degree, but cure by VT with HYZY soup was better indeed. This may provide a new experimental proof to its clinical appliance.
