Clinical And Experimental Study On Chronic Renal Failure With Total Saponins Of Panax Notoginseseng

Objective:To observe the effect of total saponins of notoginseseng(PNS) in treating Chronic Renal Failure (CRF).To study the effect of PNS on renal structure and function in renal cortex of 5/6 nephrectomized (NX) rats and explore its mechanism.Methods:1.60 patients with CRF were divided randomly into two groups with the ratio of 1:1,the experiment group and the control group.Patients in the experiment group(30 cases) were administered with PNS ,while those in the control group(30 cases ) treated with Bailing capsule.Total therapeutic course was 2 monthes for both groups,the changs of urinory proteion per 24hrs,hemoglobin,renal function,parathyroid hormone(PTH),N-actely-β-D-glucosaminidase (NAG) were observed in two groups.2.32 5/6NX rats were divided randomly into five groups:the normal control group,the model group,Bailing capsule group,the high-dosage PNS group and the low-dosage PNS group. Medicine was given to each group according to the designed concentration and dosage.The therapeutic course was 12 weeks. Proteinuria,renal function were measured ,renal tissues were examined routinedly by light microscoy. Results:1.After the treatment, the level of hemoglobin was significantly higher and urinory proteion per 24hrs was lower than before the treatment(P<0.05～0.01), while the levels of creatinine(Scr),blood urea nitrogen(Bun),uric acid(UA),PTH,NAG were not remarkable lower(P>0.05).2.PNS could significantly reduced proteinuria,increased the level of hemoglobin(P<0.05), up-regulat MMP-2 and down-regulat TIMP-2 expressions in renal,besides inhibiting the FN,ColⅣ. It retars the progression of glomerulosclerosis . Conclusion:1.PNS has a good therapeutic effect for the treatment of CRF.Its possesses such as lowering proteinuria,improving the level of hemoglobin ,ect.2.PNS had effect in protecting renal function and alleviate renal Patholog- ical injuries.The potential mechanism may be related to reducing proteinuria,improving renal function,mitigattion glomerulosclerosis through the down-regnlation of TIMP-2, FN,Col-Ⅳand the activation of MMP-2 in 5/6 NX rats.
