The Clinical Study Of Carbon Nanoparticles Mapping Lymph Node In TCM And Wesetern Medicine Combined Treatment Of Rectal Carcinoma

Objective: To identify intraoperative lymph node mapping by injection carbon nanoparticles is feasible, precise,and ascendant. To discuss the improving function of locally injection carbon nanoparticles in rectal carcinoma operations, and research the sentinel lymph node status reflects the regional node status and focused analysis of the sentinel lymph node improve staging.To identify carbon nanoparticles assisted retention-enema with Chinese medicine targeted therapy rectal cancer lymphatic metastasis is feasible.Methods: These 38 patients with rectal carcinoma from Aptil ,2007 to March,2008,were randomized into two groups,19 cases with local injection of 1ml(50mg) carbon nanoparticles suspension injection around the peripherd of the neoplasm(CNP group),another 19 case with standard lymphadenectomy only (control group).To detect and register the number of SLN and black-stained lymph nodes, compared with control group.All lymph nodes were analyzed by hematoxylin-eosin staining. sentinel lymph nodes were analyzed by serial section.Result: CNP group identified at least 1 sentinel lymph node in19 patients(100%).41 SLNs were identified. Sensitivity, accuracy and pseudo negative of the SLN biopsy were 88.9%, 84.2%, 11.1%.The number of dissected lymph nodes in each case, and in the second and the third stage, were more than that of control group respectively.Conclusion: SLN detection by injection carbon nanoparticles can precisely predict the region lymph nodes status in rectal carcinoma.It also can increase the number of dissected lymph nodes and improve the operation. Focused analysis of the sentinel lymph node increase the detection of micrometastases and may improve selection of patients for adjuvant treatment. Confirmed carbon nanoparticles assisted retention-enema with Chinese medicine targeted therapy rectal cancer lymphatic metastasis is feasible.
