OBJECTIVELearning about the cognitive degree on medication safety and the ability of understanding on dispensatory in community patients with chronic disease through survey questionnaires, and studying the medication safety risk in patients with chronic diseases; talking about the present condition of pharmaceutical care with community patients with chronic disease, and analyzing their requirement for pharmaceutical care ,so that pharmaceutical care could be carried out in community health centre, rational administration could be encouraged and medication safety of community patients with chronic disease could be ensured.Encouraging rational administration and ensuring medication safety are major contents of pharmaceutical care, and community patients with chronic disease are target population of pharmaceutical care, they have potential risk in medication safety for their taking one or more than one medicines for a long time, at the same time, the present pharmaceutical care is not perfect, medication explanation and medication safety education are not satisfied, dispensatory is too specialized to be understood, which increase the probability of drug-induced sufferings. It is helpful to aims directly to carrying out pharmaceutical care learning about influential factor on medication safety and the requirement for pharmaceutical care through questionnaires and interviews, and achieve the purpose of encouraging rational administration and ensuring medication safety.METHODS1."Questionnaires on medication information", "Questionnaires on ability of understanding dispensatory" and a deep interview were designed to investigate the condition of medication safety in community patients with chronic disease. 2.300 patients were received two questionnaires and were investigated for medication safety,who were screened with chronic diseases including hypertension,hyperlipemia and diabetes mellitus in inhabitants health check in Shengdong community, Shengli Oilfield during Nov. 2006 -Nov 2007. In order to ensure the reliability of data, all questionnaires were taken back on the spot. 300 out-patients with chronic diseases were accepted no less than 20 minutes interviews about their requirements for pharmaceutical care in chronic disease out-patient clinic,shengdong community health centre.3.Investigation data was handled with software SPSS11.5 after rejecting invalid questionnaires. Internal consistency of the scale was evaluated by calculating Cronbach's alpha coefficient, descriptive statistical analysis was understaken by calculating mean values and standard deviation of every item of patient, the differences of cognitive degree on medication safety and the ability of understanding on dispensatory in patients between different genders, different ages, different educational levels, different occupations and different medical insurance patterns were analyzed respectively by t-test and one-way ANOVA analysis, structural differences of cognitive degree on medication safety and the ability of understanding on dispensatory in community patients with chronic diseases were analyzed by explorative factor analysis.RESULTS297 questionnaires recovered (99%) were valid. Mean score of "Questionnaires on medication information" was 2.20, which showed the community patients with chronic diseases hadn't a sufficient knowledge on medication safety, especially on drug allergies, drug side effects and special population medication. Mean score of "Questionnaires on ability of understanding dispensatory" was 2.63, which showed they couldn't understand the dispensatory well, especially about the abbreviated words "OTC", "Rx" and drug specifications, function and indication of TCM, adverse drug reactions, drug interactions, contraindications and indications. There was significant difference on the cognitive degree on medication safety and the ability of understanding on dispensatory of community patients with chronic diseases between different genders, different education levels, different occupations and different medical insurance patterns. The interviews showed community patients with chronic diseases had requirements for pharmaceutical care.CONCLUSIONThe hidden danger of medication safety is present based on this investigation, knowledge on primary medication mastered by community patients with chronic diseases is limited; there is some defects in dispensatory in force, in which some phraseology is too specialized to be understood by patients. Pharmacists should assume the responsibility of explaining dispensatory, carrying out pharmaceutical care, encouraging rational administration and ensuring medication safety.
