According to the estimates of World Health Organization, there are 8.4 million new tuberculosis patients in the world each year, of which 95 percent are in developing countries. And the TB deaths each year are 2 million, of which 98 percent occur in developing countries. 75 percent of patients are between 15 and 50 years old, which brought a heavy financial burden and economic losses to these patients' family. It is estimated that from 2000 to 2020, 1) 1 million people will be infected with tuberculosis, 2) TB patients will reach 200 million, 3) at least 35 million people will die of tuberculosis.The TB epidemic situation in China is similar to the world. China is one of the 22 most TB burden countries in the world, and the prevalence of tuberculosis ranked second in the world, second only to India. It is estimated that the total number of TB patients in China is a quarter of the world, of which 80 percent are distributed in the vast rural areas. Particularly the epidemic situation in the elderly, the less, the border and the poor areas are even more serious. TB has become one of the major diseases which lead to rural poverty, restrain the economic and social development in rural. ObjectiveIn order to understand the treatment of tuberculosis patients and affecting factors, to analyze the effect of various factors in the treatment of tuberculosis patients, we surveyed the tuberculosis patients in the six counties in Shandong Province. So that we can provide a scientific basis to develop practical tuberculosis controlling measures in Shandong Province, as well as to provide some policy and advice to Shandong Province and other similar areas. The specific objectives of this study are as follows:1. To understand the basic characteristic and the family economic situation of tuberculosis patients in different areas.2. To analyze the effect on the treatment of tuberculosis patients with different demographic characteristics, areas, family size and economic conditions.3. To analyze the effect on the treatment of tuberculosis patients with different diagnosis and treatment.4. To analyze the effect on the treatment of tuberculosis patients with different compliance, supervision and management from doctors.5. To analyze the effect on the treatment of tuberculosis patients with different subjective evaluations and the response from family members.6. To provide policy advice based on the results above.MethodsThe method of stratified-cluster-random sampling was used in this study, and six counties, thirty-six towns were taken out. SPSS13.0 and EXCEL 2003 were used in this article. Descriptive analysis, Chi-Square Test, Kruskal-Wallis H test and ordinal regression were used in the process of data analysis.Main Results1. Among the tuberculosis patients who had been the end of treatment, 50.50 percent were cured, 24.15 percent completed the period of treatment, 8.98 percent interrupted the treatment and 16.37 percent failed in the treatment.2. The household income was lower among the tuberculosis patients. The annual household income was 8766 yuan. Annual per capita income was 2760 yuan. And the annual expenditure of family medical care was 4487 yuan.3. The treatment and affecting factors of tuberculosis patients:①There are more cured tuberculosis patients in Zoucheng and Jinxiang than in Wudi and Bincheng.②The higher family economic condition in the local, the better completion of treatment.③Except the patients who was discontinuity in treatment, the lower expenses before and after diagnosis, the better completion of treatment.④The completion of treatment was better among the patients who could go to take medicine and examine regularly.⑤The completion of treatment was better among the patients who had been supervised and managed by doctors through treatment. And the more rural doctors visiting, the better completion of treatment.⑥The completion of treatment was better among the patients whose medicine were kept by doctors.⑦The completion of treatment was better among the patients who had better physical condition and no anxiety.⑧Ordinal regression analysis showed that the affecting factors of treatment of tuberculosis patients were the number of rural doctors visiting, area, the family economic condition in the local, the first diagnosis, the supervision and management of doctors.Conclusions1. To ensure the effective enjoyment of tuberculosis patients for free examination and treatment.2. To play the role of village doctors fully in the management and find of TB.3. Establish a patient-centered management mechanism to ensure that patients can receive timely and effective treatment.4. Play the role of doctors fully in the supervision and management to improve patients' compliance and ensure the completion of treatment.5. To strengthen health education, ensure the timely treatment of patients and save medical costs.6. To focus on rural areas, promote the prevention and treatment of TB.
