Objective Primary open angle glaucoma(POAG) is a serious blind causative oculopathy which is characterized by high intraocular pressure(IOP).However,the pathogenesis of high IOP is unclear until now.Generally speaking,it is associated with resistance to aqeous humor outflow caused by the abnormal anatomic structure of chamber angle.We observed the histopathological characteristic of Schlemm's canal and collector channel in POAG in order to approach the factor which affected the outflow of aqueous humor.Methods There are three diadynamic criteria of POAG:high IOP,damage of optic disc and defect of visual field.While two of them are positive and the chamber angle is open,it's POAG definitely.In this study,we collected 85 clinical definite POAG eyes and 32 normal eyes.There were 30 eyes respectively in which could be found the debouch of Schlemm's canal to collector channel.The tissues from trabeculectomy of 20 clinical definite POAG patients(30 eyes) were used as experimental group(EG),the mean age was 59.47±10.86 years old(range from 43 to 77 years old);there were 10 males(13 eyes) and 10 females(17 eyes) in EG;patients with different states of an illness were included,like different high IOP,different damage of optic disc and visual field.30 eyes which had normal appearance, equivalent age(15 persons,mean age 59.20±12.75 years old) and no any known ocular disease were used as control group(CG).All the chamber angle tissues and limbal tissues were observed by transmission electron microscope,followed by coefficient correlation statistics between the degree of pathological change and age,preoperative IOP,cup disc ratio(C/D) or visual field defect.Results In EG,27 out of 30 eyes appeared fingerprintlike electro-dense materials in the debouch of Schlemm's canal to collector channel.The number of them had positive linear correlation with C/D(r=0.66,P<0.001);and has no correlation with age,preoperative IOP or visual field defect(r=0.03,P > 0.50;r=-0.24,0.20 < P <0.50;r=0.30,0.10
