Clinical Study On Treating The Diabetic Nephropath Associated With Hypercoagulabale States With The Method Of Invigorating The Spleen, Tonifying Kidney, Eliminating Phlegm And Dissipating Blood Stasis

Objective : Diabetic nephropathy is one of common and serious complications of diabetes mellitus. Recent study shows that hypercoagulable states plays an important role in the occurrence and development of DN. In this research,to observe the therapeutic efficacy jiang-zhi-kang-ning decoction in treating diabetic nephropathy associated with hypercoagulabale states,and explore the protective effect and its mechanism of this decoction on the development of DN.Method: 57 patients with clinical stage of diabetic nephropathy associated with hypercoagulabale states were divided into 2 groups:the treatment group ( 28 patients ) was treated with jiang-zhi-kang-ning decoction to ameliorating hypercoagulabale states.The control group (29 patients) was treated with jiao-gu-lan tablets.Both of the groups were given basic treatments at the same time.All the patients were treated for 3 months.The TCM syndromes were observed.Moreover,D-dimer,24hupq,blood lipid and the index level of blood rheology were measured and compared before and 3 months after starting the treatment.Result: The recipe in treating diabetic nephropathy ameliorated the clinic symptoms of the patients obviously, the difference was significant compared to the control group (P<0.05) .The levels of D-dimer lowered in the control group (P<0.05), and the changes of D-dimer before and after treatment showed significant difference statistically in the treatment group. Meantime,not only the bloodlipid and the index level of blood rheology all benefited from the treatment.Conclusion:(1) The total effective rate of the treatment group was 75.0%,while it was 65.5% in the control group,so the clinical effect of the treatment group was better than that of the control group.Jiang-zhi—kang-ning decoction can improve symptoms of patients with clinical-tage DN.( 2 ) Jiang-zhi-kang-ning decoction can amend hypercoagulabale states,improve abnormal blood rheology and microcirculation, defer development of glomerulosclerosis.(3) Jiang-zhi-kang-ning decoction can obviously decrease urinary protein excretion,regulate glomerular filtration function,protect nephron and can help to slower the progress of diabetic nephropathy.
