Objectives :The purpose of this research was to assess the association between the metabolic syndrome (MetSyn) and ischemic stroke events by meta-analyses of longitudinal studies, in order to provide evidence for prevention of ischemic stroke.Methods: Search MEDLINE (1966-2008) was conducted to identify prospective studies that examined the association between the metabolic syndrome and risk of ischemic stroke. Information on sample size, participant's characteristics, metabolic syndrome definition, follow-up duration, and endpoint assessment was abstracted, then we calculate the RR through the comprehensive meta analysis soft 2.0.Results: We found 11 eligible studies which included11 cohorts (inception 1971 to 1997) and 72598 individuals. Random effects meta-analyses showed MetSyn had a relative risk (RR) of ischemic stroke 2.105 (95%confidence interval [CI] 1.710- 2.590, P=0.000).The association was stronger in women than men (2.532 vs. 1.755). In community -based studies, individuals with the metabolic syndrome, compared with those without, had an increased incidence of ischemic stroke (RR 2.279; 95%CI 1.745- 2.976; p=0.000); in patients with type 2 diabetes mellitus, the metabolic syndrome was associated with ischemic stroke (RR=1.775; 95%CI 1.047 - 3.009; p=0.033).Conclusion: This Meta- Analysis strongly suggests that people with the metabolic syndrome are at increased risk of ischemic stroke in community population and patients with type 2 diabetes mellitus, the association is stronger in women than men.
