The Effect Of IGF-Ⅱ In Cervical Cancer Screening

Objective: To determine the levels of insulin-like growth factor - II (IGF-II )in blood serum in patients of cervical intraepithelial neoplasia (CIN) and cervical cancer to investigate whether it is suitable for cervical cancer screening, and in order to find a more simple and economical method in its early diagnosis, which would be helpful for reducing cervical cancer incidence rate.Method: The blood serum samples were selected randomly from the women between 19 and 59 years, who are normal in intelligence ,and all of samples were collected in the clinic service of the Second Hospital of HeBei Medical University from December 2006 to August 2007, Every patient received a Thinprep cytologic test and pathology inspections were carried on in those patients with abnormal results. The cases were devided into two groups: control group and experimental group according to the TCT and pathology result. The experimental group including CINⅡ, CINⅢand cervical cancer patients, while those women with normal TCT, chronic cervicitis and CINⅠbelongs to the control group. The enzyme-linked immunosorbent assay (ELISA) was used to assay the levels of IGF- II in serum, and to take the pathology results as reference standard. Results were analyzed by SPSS 13.0 with use of t test and Chi-square test; For these analyses, P<0.05 was considered statistically significant. Through SPSS analysis, we can get a positive predictive value, based on which the sensitivity, specificity , positive predictive value and predictive value were calculated.Results:(1)176 serum samples were collected, including 45 normal women, 33 chronic cervicitis patients and squamous metaplasia, 30 CINⅠ, 34 CINⅡ, 9 CINⅢand 25 cervical cancer. And among the total CIN 73, 44.48%; high cervical intraepithelial neoplasia(CINⅡ~CINⅢ) 43, 24.43% and cervical cancer 14.20%. (2)The minimum age was 23 years old, the maximum age was 59, and the average age was 38.9±8.2. This investigation demonstrated that most of the patients with highly cervical pathological change are 30~49.(3)There were 155 passive smoker(88.07%), but no active smoker, among the 176 patients, while there were 62 passive smoker(92.65%) among 68 highly cervical pathological change cases. It indicated that the patient of highly cervical pathological change between 30~49 was more than another two age stages. (4). In the investigation, we divided the educational degree to three stages,primary,middling and high. In three stages, the disposition that patient of highly cervical pathological change was 37,27,9, thus we could considered higher education, lower morbility of highly cervical pathological changes group(P<0.01). (5) the abnormal effects of TCT was 96, 54.55% of the total. The reference standard was pathologic diagnosis, the sensitivity, specialty positive predictive value ,and negative predictive value of TCT reached 97.05%,71.30%,67.35%,97.46% respectively in diagnosis of cervical pathological changes. (6)The serum IGF-Ⅱof experimental group was in Gaussian distribution ,and there was not significant difference among normal women, chronic cervicitis patients and CINⅠpatients(P>0.05). The mean IGF-Ⅱof serum in experimental group was 481.00±71.58 ng/ml. The mean IGF-Ⅱserum of in experimental group was 481.00±71.58 ng/ml , and there were significant differences between them ( P<0.01 ) (7) 95 %confidence interval in control group was 461.27~500.73 ng/ml; 95 % confidence interval in experimental group was 461.27~500.73 ng/ml ,and according to which we got the sensitivity, the specificity, the positive predictive value and the predictive value of IGF-Ⅱin they were 86.76%,71.30%,65.56%和89.53% respectively.Conclusion: IGF-Ⅱis one of the important factors that tumor tissues need, and circulating IGF- II may be a available target for early cervical cancer diagnosis . IGF- II in the blood serum correlate with cervical cancer closely .Maybe it is helpful for cervical cancer Screening.
