Objective: Peripheral nerve injury is a commonly-encountered disease, especially injury ofsciatic nerve. the recovery of the nerve functions is very important,it effects the life quality of the patients.How to promote repair of injured nerve is always the important research task in clinical and basic works. Muscle atrophy and lost function due to denervation after peripheral nerve injury.Nerve regeneration can not by mitosis of nerve cell. Neurofunctional recovery just by regeneration of axons and myeline sheaths . Peripheral nerve injury and its repair is a complex pathophysiological course . Reasonable treatment , microenvironment and local blood supply effect repair peripheral nerve injury.Our experiment used SD rats. To investigate whether the effect that the repairation and the recovery of Injury of sciatic nerve function is used by ERSVV is better than alone by Anti-imflammation-analgesic Injection ,in accordane with the model of CCI,and to suPPly the foundation for clinic therapy.Methods : Eighty male Spague-Dawley rats weighing 260 to 300 g were divided randomly into four groups: .A was the sham group,B was the CCI group , group C was the Anti-imflammation-analgesic Injection group,group D was the combined drugs therapy(Anti-imflammation-analgesic Injection+ERSVV). The rats were anaesthetized with 10% chloral hydrate(300mg/kg),made the middle femoral incision. The sciatic nerve of the rats in the A group were exposured only, no ligation. Rats with right sciatic nerve been ligated and placed the epidural catheter were used as group B,as group C,group D.This three group were injected with water for injection by the epidrual catheter.After CCI fourteen days, rats pain-related behavioral response was irritability.Sciatic nerve was extracted and observed with light microscope. The sciatic nerve were edema and axon number reduced and medullary sheath were disaggregated. CCI model rats were successfully constructed. On day 14, group C were injected with Anti-imflammation-analgesic Injection (5mg/ml Dexamet- hasone 0.05ml+2%dolicaine0.05ml+0.5mg/mlVitamin B12 0.05ml+water for injection0.05ml) stopped to get water for injection .GrouP D were injected with Anti-imflammation-ana- lgesic Injection and ERSVV(5mg/mlDexamethasone0.05ml+2%dolicaine0.05ml+0.5mg/mlVitamin B12 0.05ml+40U/ml ERSVV0.05ml) stopped to get water for injection. Thermal withdrawal latency(TWL) and mechanical withdrawal threshold(MWT) were measured on preoperative day 1, postoperative days 1, 5, 7,9,14, 17,20,24 and 28. Measure nerve conduction velocity and action potential on the 4th and 7th day after CCI 14 days and 28 days ,then materals were gained for observation of ultrastructure of sciatic nerve , axonal amount,and measure thickness of myelin sheath.Results: There was no change in the thermal withdrawal latency and mechanical withdrawal threshold in the sham rats during the observed period for 28 d after the sham operation. The CCI induced the development of thermal hyperalgesia and mechanical allodynia represented with decreases in the thermal withdrawal latency and mechanical withdrawal threshold in the hindpaw ipsilateral to the CCI.Group Breached to level of 55% and 20% of normal values on postoperative days 5 and 7 ,and continued to the postoperative days 28 observed. Compare with group B, C and group D TWL increase clearly(P<0.05) on day17,20,24, 28 .MWT increse clearly on day 24,28(P<0.05).Compare with group C,TWL increse clearly on day 24,28,MWT increse clearly on day 28.The studies of nervous electrophysiology show that compare with A,B,C,D group MCV and APT significantly decreased(P<0.05) .Compare with Group B,MCV and APT of group C and group D increase clearly on day 28. Compare with C, D group increase clearly. After treatment on day 28 both groups of were remarkably highter than those of on day 14. (P<0.05).The morphometric analysis: compare with A, axons counts and myelin sheath thickness of B,C,D group significantly decreased(P<0.05) .Compare with group B, axons counts and myelin sheath thickness of group C and Group D increas clearly on day 28.Compare with C, group D increas clearly. After treatment on day 28 both groups of were remarkably most than those of on day 14. (P<0.05).Histopathological observations : The histological observ- ation under light microscope that the nerves of Sham group is integrated and structure rules .On the 14th , the nerves of B,C, D group have severe injury. On the 28th ,group B have small nerves grow but there is no medullary sheath.The medullary sheath of group C thicken .group D we can found small nerves with medullary sheath.Electron microscopic observation :A group sheath plate is rules. On the 14th ,the nerves of B,C,GrouP D have severe injury , mitochondria and microtubule of this three group became less.On the 28th , sheath plate of group C became mild hyperplasia. Mitochondria and microtubule of Group D became more ,sheath plate became rule.Conclusion: Therapy the chronic neuropathic pain through combination with extract from rabbit skin inflamed by vaccinia virus for injection have better effect .
