Purpose:Clinical N0 stage papillary thyroid carciNoma(PTC)is only a clinical concept,there would be some patients with this malignancy have had the neck lymphatic Node metastasis which was confirmed by postoperative pathology.It is well known that PTC has a high rate of neck lymphatic Node metastasis and the apparent potential to matastase to regional lymphoNodes.The literature reported that about 60.9%of PTC patients had neck lymphatic Node metastasis at the first treatment.The results of studies have confirmed that the high expressions of CyclinD1,c-Met may play an important role in the development of PTC.The study mainly aim at verifying the biologic molecular markers related to the cervical lymphatic Node metastasis of PTC at No stage,measuring the level c-Met and CyclinD1 expressioris at transcriptional level by way of RT-PCR and FQ-PCR, exploring whether the abNormal expression of mRNA of these two genes is relative to the sex,age and the neck lymphoNode metastasis of No stage PTC patients or Not, the genetic evidence of the etiology of PTC,the relationship between the abNormality of these genes mRNA expression and the clinical pathologic appearances,providing the primary clue to the early molecular pathologic diagNosis and the strategy of treatment of PTC patients.Material and Methods:Sixty-six fresh surgical specimen of benign and malignant of thyroid gland tumour were collected,Of which thirty are Non-cancerous thyroid tissues(goiter,adeNoma,Hashimoto disease and some paraneoplastic tissues)and thirty-six are papillary carciNoma at clinical No stage.All of them with No history of radiotherapy and chemotheraphy.Total RNA were extracted from the surgical specimen,and the presence of c-Met and CyclinD 1 were assessed by RT-PCR and FQ-PCR assays.The targeted DNA fragment of c-Met and CyclinD1 were valuated by 2%agarose gel electrophoresis.Of the experiment,the positive control group in the experiment consisted of thirty-six papillary thyroid carcinoma,of which twenty cases were confirmed the neck lymphoNode metastasis by routine hematoxylin-eosin-stained section of PTC postoperatively and the negative control group consisted of thirty Non-cancerous disease patients.The results of relative quantitative assays of these two genes mRNA expressions were analyzed according to the patients' sex,age and lymph Nodes metastasis.The statistical procedure was performed by way of ANOVA(Analysis of Variance),independent -samplesT test and relationship analysis. Rusults:The results showed that in PTC group,20 cases had neck lymphic Node metastasis,the neck lymphoNode metastasis rate is 55.56%.The transcriptional level expression of c-Met and CyclinD1 in the pN+ group is higher than that in the pN-group(p<0.05),the expression of c-Met in male group is higher than that in female group,in under 45 years old group is higher than that in over 45 years old group,while the expression of CyclinD1 has No significant differences in these above groups.The expression of c-Met in malignant group is higher than that in benign control group,while CyclinD1 is of No obviously differences in both two groups.No correlationship was found between the expressions of c-Met and CyclinD1 mRNA in malignant group.Conclusion:The experimental results showed that the high expression for c-Met and CyclinD 1 demonstrated existence of micrometastasis in the lymphoNode of neck of papillary thyroid canciNoma.The high expression for c-Met in the thyroid cancer was correlated with patients' sex and age.The expression level of c-Met in malignant group is higher than that in Nonmalignant control group,while the expression of CyclinD1 between the two groups is of No significant difference.So,it may be concluded that these abNormal changes of CyclinDI expression may be one of early stage molecular evidences in the lymphoNode metastasis procedure of PTC.Therefore, it may be possible to discover the micrometastasis foci on the patients' neck through testing the c-Met and CyclinD1 mRNA using RT-PCR and FQ-PCR method.
