V-Y Tendinous Flap For Treatment Of Old Achilles Tendon Rupture And Its Related Anatomic Study

Objective: To evaluate the surgical method and resultes of chronic rupture of the Achilles tendon using V-Y tendinous flap,and make an study of the related anatomy,so that to provide a antomic basis and reference for surgeryMethods: 1 .Retrospective analyzed thirteen cases with chronic rupture of the Achilles tendon that treated with V-Y tendinous flap during 2003.2-2007.7 in our hospital and assess the results of surgical treatment by the Arner - Lintholm's evaluation criterion. 2.The superficial vein of the soft tissue surrounding the achilles tendon,sural nerve,gastrocnemius,soleus and achilles tendon were observed and measured on 30 adult lower limb specimens that without distinction of men and women. Make simulation V-Y tendinous flap on 10 specimens which divided into five groups with the achilles tendon defect in 3cm, 4cm, 5cm, 6cm and 7cm respectively.Results: 1All 13 Cases were followed up from 7～60 months, average of 24 months,and 9 cases of the 13 had excellent results and 4 cases had good results, with the excellent and good rate being 100%. 2.The V-Y tendinous flap surgery is easy to make when the achilles tendon defect from 3cm to 6cm but more difficult when the defect reach 7cm.Conclusion: The V-Y Tendinous Flap is a simple and good method for treatment of chronic rupture of the achilles tendon and it is mainly suitable for defect of the achilles tendon from 3 cm to 6cm and can use carefully at 6-7cm but not suitable for the defect more than 7cm.
