ObjectiveWe have analyzed the single effect of environmental factors including smoking, drinking wine, eating live fish, major drinking water, disease association condition and so on, DNA repair gene XPC 8, 15 exons Ala499Val SNP, Lys939Gln SNP, and the gene-environment, gene-gene interaction on the development of primary hepatic carcinoma (PHC) to provide some clews for the further study of etiology, and initially investigated the method of the case only study as well.MethodsGuangdong Fusan Shunde has been selected as the study spot because its incidence of PHC is very high. 81 PHC patients as cases are the Shunde residents who had hospitalized and been registered in region's hospitals. We have applied 1:1 pair matching case-control study, choosing the persons as controls who had dwelled in Shunde for more than 10 years during the same period, having no blood relationship to the patients and been matched by sex, village, age at diagnose(±3 years). Unified questionnaire has been used to collect environmental factors which include society demography information, lifestyle, eating habit, major drinking water, history of the development of liver cancer correlated disease and familial inheritance, and so on. Based on knowing the fact, part of patients in Xingtan town of Shunde had been investigated by investigationers who had been unitilly trained in 2007.1. On the basis of information collected, we amended questionnaire and then had carried out the formal investigation during 2007.1~2007.12, and the periphery venous blood of the cases and controls had been collected. By applying molecular biology technology, we have detected nucleotide excision repair gene XPC 8, 15 exons genotypes, entered data with EpiData3.1, and analyzed them with SPSS13.0, utilized conditional logistic regression and case only study to analyze the relationship among environmental factors, gene and the development of PHC, and probed the interaction of gene-environment and gene-gene as well.Results1 environmental risk factors of primary hepatic carcinomaThe variables ultimately have entered the conditional multiple factors logistic regression analysis are HBsAg condition and total quantity of drinking wine, their OR is 6.113(95% CI:1.948~19.180),3.557(95% CI:0.932~13.572).2 the effect of HBV infection and other environmental risk factors interaction on liver cancerThe interaction OR of HBV infection and eating live fishes, gruel with live meat or fish, drinking well water is 8.397(95% CI:2.122~33.236), 6.156(95% CI:1.872~20.241), 5.677(95% CI:1.415~22.782); the synergistical OR of HBV and HCV infection is 11.742(95% CI:1.412~97.618).3 the effect of XPC gene Ala499Val,Lys939GIn polymorphisms and environmental interaction on liver cancerThe OR of non-alcohol user who carries at least a mutant gene of Ala499Val is 1.567(95%CI:0.449~5.470),and only drinking wine ones association with liver cancer is 4.268(95%:1.169~15.584), but the OR of those alcohol user who carry at least a mutant gene of Ala499Val is 1.785(95%:0.573~5.563).Only with HCV infection, ones OR is 8.799(95%CI:1.076~71.940), and not only HCV infection but also carry at least a mutant gene of Lys939GIn, ones OR is 1.56(95%CI:0.425~5.728).4 exploration of case only controlThe consequence of gene-environment of case only control is close to case-control study, and it could replace the latter when the blood of control is difficult to be collected.Conclusions1 HBV infection, drinking wine are the major risk factors of primary hepatic carcinoma in Guangdong Shunde.2 Carry C/T or T/T genotypes of XPC genes 8 exon Ala499Val could lower the risk of people who drink wine to get liver cancer; and with 15 exon Lys939GIn A/C or C/C genotypes could also decrease the ones with HCV infection to suffer liver cancer.3 When the blood sample is hard to be collected, case only study could replace case-control study to evaluate the interaction of gene and environment.
