Objective To evaluate clinical efficacy of laser in situ keratomileusis (LASIK) and Q-value guided aspheric laser in situ keratomileusis for myopia correction in the aspects of visual quality. Methods The study was used for 144 eyes of 72 patients who were going to accept LASIK or Q-value guided LASIK surgery in ChangHai hospital from September to December in 2007. The enrolled criteria involved spherical equivalence no more than -8.00D, the astigmatism power no more than -3.00D,the best corrected visual acuity (BCVA) 0.80 or more, the corneal contact lens removed for at least 2 weeks, and at least 3 months followed-up.The patients were divided into standard LASIK(controlled group) group and Q-value aspherical LASIK group (experimental group). Preoperative and postoperative examinations included uncorrected visual acuity (UCVA) and best corrected visual acuity (BCVA), refraction, contrast sensitivity with or without glare, mesopic contrast sensitivity, K\Q value, corneal topograghy, the examinations related dry eye, and corneal thickness. The questionaires visual performance, including satisfaction and symptoms were administered preopertively and 1-month, 3-month postoperatively. Results 1. The postoperative contrast sensitivity of each space frequency in both groups decreased . The contrast sensitivity after both surgeries mostly recovered in three months. The postoperative glare sensitivity of each space frequency in both groups decreased . The glare sensitivity after both surgeries mostly recovered in three months. There is no significantly different between 2 groups. 2.On the 1st month ,control group and experimental group decreased obviously at the 3rd grade without glare ,and control group decreased more obviously .Both groups have statistical significance. On the 3rd month, the contrast sensitivity of both groups increased than the results of the 1st month. Experimental group increased more obviously than that of control group at the 3rd and 4th grade. Both groups are different significantly. On the 1st month ,the contrast sensitivity of control group and experimental group decreased obviously at the 4th grade with glare ,and control group decreased more obviously .Both groups have statistical significance. On the 3rd month, both groups increased than the results of the 1st month. Experimental group increased more obviously than control group at the 3rd and 4th grade. Both groups are different significantly.3.Q-value: Postoperative Q-value for the Q-guided group acquired significantly less oblation when compared to the standard ablation group. Both groups are different significantly. Conclusions Both Q-value guided aspheric ablation and standard ablation LASIK surgeries show very good visual efficacy after 3 months of follow-up. The Q-value aspheric group appears to keep cornea aspheric and help patients get better night visual quality than that of standard surgery ,but its long efficacy needs more cases, more evaluation index and more time follow-up to prove.
