Introduction Among all lacrimal gland tumors, the pleomorphic adenoma, adenoid cystadenocarcin, pleomorphic adenocarcinoma and adenocarcinoma were the most common ones. According to the latest WHO's classification, pleomorphic adenoma of lacrimal gland is the common benign tumor of Lacrimal gland tumors. And 20% of all pleomorphic adenomas can develop into malignant tumors eventually. The liability to relapse is its major biological property. Though the mechanisms of relapse and the causes to the pleomorphic adenoma of lacrimal gland are still unidentified. It has been reported that Survivin and p53 may play a great role in the occurrence of tumors. We studied the expression of Survivn and p53 in the pleomorphic adenoma of lacrimal gland and the relationship between their expressions with the clinicopathologic variables to explore the independent factors in the prognosis of the pleomorphic adenoma.Objective To study the relationship between Survivin expression and p53 expression with the clinicopathologic variables and whether the expressions of both survivin and p53 is related to the relapse of pleomorphic adenoma of lacrimal gland.Method Surgical and pathological verified pleomorphic adenoma cases during 1985-2007 were taken. All clinical and pathological data were individually reviewed. Summarize the clinical and pathology features. Expression and correlation between p53 and Survivin were examined by using immunohistochemicalthod in 46 cases of pleomorphic adenoma and 6 cases of normal lacrimal gland. Univariate Kaplan-Meier model and multivariate Cox regression model were used to find the independent factors of relapse of the pleomorphic adenoma of lacrimal gland .Results Survivin expression and p53 expression were negative in normal lacrimal gland .Survivin expression and p53 expression were positive in 15 of 46 cases (32.6%) and 11 of 46 cases(23.9%) respectively. Overexpression of Survivin and p53 were not related to age,sex,tumor size ,and onset time. Univariate analysis showed that Survivin expression ,p53 expression and coexpression of Survivin and p53 were significantly correlated with the relapse of the pleomorphic adenoma. Multivariate analysis with the Cox regression models showed that the expression of Survivin was significantly associated with relapse of pleomorphic adenoma.Conclusion The abnormal expression of Survivin and p53 gene suggested that they may play a role in onset and progression of the pleomorphic adenoma. Multivariate analysis with the Cox regression models showed that Survivin act as an independent factors of the relapse of pleomorphic adenoma of lacrimal gland.
