A Clinical Analysis Of Etiology,Seizure Type, Et Al And Prognosis Of 300 Cases Of Epilepsy

Obstractive:We analysed gender,age of onset,course of diseases,etiology,seizure type, electroencephalograms(EEGs),neural imaging examination,the drugs therapy,serum concentration of antiepileptic drug(AED) and compliance in this article,and develop models to assess the affecting factors of prognosis in patients with epilepsy.Methods:1.Collect of 400 cases of epilepsy in the First Hospital of Shanxi Medical University epilepsy-outpatient from April to October in 2007,all of them were definitely diagnosed epilepsy; 2.We mainly analysed gender,age of onset,course of diseases,etiology,seizure type,EEG,neural imaging examination,the drugs therapy,drugs serum concentration and compliance of epileptic patients;3.Analyze retrospectively and summarize these clinical data with the Cox regression analysis by SPSS 13.0 software.Result:Three hundred patients(180 males and 120 females,a male to female ratio of 3∶2) were selected from the 400 epileptic patients.Age of onset ranged 2 years to 62 years,2 years to 14 years had 131 cases(43.67%),and above 14 years had 169 cases(56.33%).Course of diseases ranged six months to forty years.The incidence with significant incentives were 118 cases(39.33%).The most maily causes were brain trauma and perinatal trauma.Only one seizure type had 171cases:generalized tonic-clonic seizures(GTCS) were present in 163 cases. Generalized tonic-clonic seizures with other seizures were present in 110 patients.19 cases had three seizure types or more.Neural imaging examination were abnormal in 58 cases.EEG of the 284 cases(94.67%) had repeatedly paroxysmal sharp or polyspike slow wave there.As far as drug therapy,216 patients(72%) insisted on a single antiepileptic drug(AED) all along.Serum concentrations of AED of 7 patients weren't measured;259 cases were normal.Low compliance was present in 37 patients(12.33%).After standard drug therapy,194 patients(64.67%) had at least two-year remission period and EEG was with significant improvement;Seizure frequency of 61 cases(20.33%) reduced more 50 percent and EEG was only partial improvement;Seizure frequency of 45 cases(15%) reduced less 50 percent and EEG was without significant improvement.Course of diseases,etiology,seizure type,neural imaging examination and single or multi- drugs therapy are affecting factors of prognosis.Conclusion:Epilepsy is a syndrome that multi-factors cause brain function abnormity.The patients who have long course of diseases,definite etiology,multi- seizure type,neural imaging examination deviant will have a less prognosis.According to international standard,as quickly as possible examining and therapy and single drug can control 80%of seizure.Brain trauma and perinatal trauma are the most common causes of epilepsy seizure.All of us should take an active part in announcing the knowledge of epilepsy.
