Objective To investigate the expression of costimulatory molecules 4-1BB and 4-1BBL in patients with Henoch-Schonlein purpura(HSP) and explore their possible significance.Method 34 HSP patients as research objects were devided into two groups:the first group is 13 Henoch-Schonlein purpura nephritis(HSPN)patients; the second is 21 NO-HSPN patients and the concrol is 20health children. Applying reverse transcription-polymerase chain reaction(RT-PCR) and flow cytometry(FCM) technique, use PHA, DEX as intervention to quantitatively analyze 4-1BB and 4-1BBL expression on PBMC (peripheral blood mononuclear cells) of 34 HSP patients. Relation between 4-1BB,4-1BBLand some clinical information analyse were also analyzed.Results Compaired with normal controls,the expression of 4-1BBmRNA,4-1BBLmRNA and Positive Expression Cell Rate(PECR) increase obviously in HSP especially in HSPN. The expression of 4-1BBmRNA , 4-1BBLmRNA Positive Expression Cell Rate(PECR) in all objections both increase obviously especially in HSP after add in PHA. The expression of 4-1BBmRNA , 4-1BBLmRNA Positive Expression Cell Rate(PECR) of the control in PBMC decrease little but decrease obviously in NO-HSPN and HSPN after add in Dex.Relation with clinical information :Expression of 4-1BBmRNA,4-1BBLmRNA and Positive Expression Cell Rate(PECR)in all patients with HSP has no significantly correlation with the leval of IgA in the blood.The expression of 4-1BBmRNA,4-1BBLmRNA and Positive Expression Cell Rate(PECR) of 14 patients with HSP whose urine microalbumin within 24 hours(24hUMA) is elevated are significantly higher than that of the other patients whose 24hUMA is normal, The expression of 4-1BBmRNA,4-1BBLmRNA and Positive Expression Cell Rate(PECR) of 7 patients with higher 24hUMA in the NO-HSPN group were higher than that of other 14 patients.and there were positive correlation between the expression of 4-1BBmRNA,4-1BBLmRNA and Positive Expression Cell Rate(PECR) and 24hUMA In these 14 patients with higher 24hUMA. The expression of of 4-1BBmRNA,4-1BBLmRNA and Positive Expression Cell Rate(PECR) of 14 patients with HSP whose urineβ2-microglobulin(β2-MG) is elevated are significantly higher than those of the other patients whoseβ2-MG are normal, The expression of 4-1BBmRNA,4-1BBLmRNA and Positive Expression Cell Rate(PECR) of 7 patients with higherβ2-MG in the NO-HSPN group were higher than that of other 14 patients.and there were positive correlation between the expression of 4-1BBmRNA,4-1BBLmRNA and Positive Expression Cell Rate(PECR) and 24hUMA In these 14 patients with higherβ2-MG.Conclusion The abnormal expression of 4-1BB and 4-1BBL may play a critical role in the pathogenesis and development of HSP.Blocking the costimulatory pathway appears to be a promising approach of the therapy of HSP. Having positive correlation with 24hUMA andβ2-MG, 4-1BB and 4-1BBL may be utilized as an immunological marker to evaluate the condition of patients with HSP and detect renal injury at earlier period.
