Ischemic cerebralvascular disease is very common disease with quite high mortality and morbidity. It's important to be early detected and intervened therapy. Although the mechanism and pathogenesis are still not quite clear. But it's well known that cerebral artery stenosis (especially Ml segment of middle cerebral artery, MCA) plays an important role in it. Even though, most stenosis could be stable, deterioration of stenosis will make patients to be high risk of stroke onset. It's necessary to screen early, start treatment as early as possible and follow up regularly on the patients with MCA stenosis.Diagnosis of MCA stenosis is depending on Digital Subtraction Angiography (DSA), but due to invasion and hospitalization of patient, it's obviously to limit DSA to be routine examination method. Transcranial Doppler (TCD) is an non-invasive, economy, easy operation and safe method to dynamic assess the blood flow of cerebral artery. Previous reports showed that TCD has the high sensitivity and specificity in detection of MCA stenosis.For evaluation of TCD role in detection MCA stenosis, 116 hospitalized patients were retrospect analyzed. Subjects prescreened by TCD and DSA were performed 7 days later. DSA results were choice as standard control. The following areas such as sensitivity, specificition, false positive and negative were parameters for comparing between TCD and DSA. The results are promising: sensitivity, specificition, false positive and negative were 81.52%, 93.57%, 10.71% and 11. 49%. No statistical significant difference exists between those two methods to identify MCA stenosis . It showed that TCD is very accurate assay to diagnose MCA stenosis. For the patients with ischemic cerebralvascular disease, TCD could be prescreen method for DSA, one of parameter for evaluation and follow up before & after intervention. For the patients with MCA stenosis, TCD could be a useful method for regular assessment and follow up evaluation. For the high risk population of ischemic cerebralvascular disease, TCD should be one of routine items in medical examination.
