[Objective] To study on differences of clinical and histopathologic features between ischemic colitis (IC) and ulcerative colitis (UC) which can make for the differential diagnosis and treatments.[Methods] Comparative analyses are focus on the clinical and histopathologic data of ischemic colitis patients (20 cases) and ulcerative colitis patients (30 cases), such as age, course of diseases, underlying diseases, symptoms, colonoscopic and histopathologic findings.[Results and Conclusions] IC patients are elder than UC patients, and tend to have history of cadiovascular diseases or abdominal surgery. The typical clinical manifestations of IC are sudden onset of abdominal pain, hematochezia and diarrhea, but UC usually presents with abdominal pain, diarrhea, bloody mucopurulent stool and Lijihouchong. Colonoscopy shows that longitudinal ulceration is principally on the mesenteric aspect, the segmental lesions usually involve only 1/4~1/2 of the perimeter of the lumen, and the boundary of lesions are clear in IC patients; on the contrary, the ulcer of UC is usually map-like, and the pseudopolyp is another colonoscopic feature of UC. Dilation and hyperaemia of vessels, severe edema of mucosa are usually presented in IC, and the thickening of vascular wall is the histopathologic characteristic of IC; however crypt abscesses usually appear in UC.
