Discussion Between Ventricular Arrhythmias In Chronic Heart Failure And Zi Wu Liu Zhu Time Rule

ObjectiveTo study circadian rhythm of different kinds of ventricular arrhythmias in chronic incongestive heart failure patients.To investigate the correlation between autonomic nervous lesion and cardiac functional grading.Discuss the link between Zi wu liu zhu time rule and circadian rhythm of ventricular arrhythmias in the same patients.To judge the interrelation for the holistic concept of traditional medicine and heart function testing index with theoretical model.MethodAnalyze heart rate variability(HRV)in time-domain index and frequency-domain index and different kinds of ventricular arrhythmias with Holter among 181 samples of coronary artery disease with chronic incongestive heart failure patients,which includes premature ventricualr beat(PVB)and PVB-pair,ventricular tachycardia(VT).Compare HRV in time-domain index and frequency-domain index of different cardiac functional gradings and TCM classification.Disscuss on the relationship between time-domain index and New York Heart Association(NYHA)cardiac functional grading.Using circle analysis by PEMS3.1 to demonstrate circadian rhythm of different kinds of ventricular arrhythmias in diverse cardiac functional gradings and TCM classifications,then confer the characteristics of them.Results1 Based on the circle analysis,analyze the average angle in the circle recording of ventricular arrhythmias and make hypothesis testing, concluding the peak time is 0:30:00～01:00:00am,Zi hour,P＜0.05,which means statistical significance in coronary artery disease with chronic incongestive heart failure patients.The same as TCM classification: excess and syndrome includes excess and deficient.2 SDNN is significant deviation from coronary artery disease with chronic incongestive heart failure samples to healthy samples,P=0.000.There shows no significant difference of HRV in time-domain index and frequency-domain index of different cardiac functional gradings and TCM classification in general,P＞0.05.Only SDNN,SDNN index and PNN50 have statistical significance when comparing cardiac functional gradingⅡandⅢ.ConclusionCoronary artery disease with chronic incongestive heart failure patients happens that sympathetic nerve activity increased,vagal tone decreased, autonomic nerve regulation of the heart attenuated obviously and vagus nerve function damage.HRV decreased with no obvious correlation NYHA classification.Concluded that no significant difference of HRV in time-domain index and frequency-domain index of different cardiac functional gradings in general,P＞0.05.This study confirms that,for circadian rhythm of ventricular arrhythmias in chronic heart failure(CHF),the peak timg is Zi hour,which conforms to TCM chronomedicine theroy that "the cardiac patients feel surgeless in the morning,day is better while night is worse" This provides a new theoretical and chronotherapy way- "following the prognosis of a disease" for clinical treatment.The new way provides guidance on working out for the prevention and treatment of ventricular arrhythmias of CHF.
