Clinical Study On The Invigorating Spleen To Remove Phlegm Of TCM To Pulmonary Atelectasis Caused By Pulmonary Lobectomy

Objective:Pulmonary lobectomy by exploratory thoracatory is one of main treatments of pulmonary disease including lung cancers,but this operation can aslo reduce post operative pulmonary atelectasis,especially to the old people. Many kinds of cardiopulmonary complications were easily occurred ,which are caused by pulmonary atelectasis,this situation increase patients 'distress,prolong the restoration time and increase the medical cost,and even endanger patients' life.Currently,there are many ways to prevent postop pulmonary atelectasis;using chemical medicine to expectorating phlegm is the most popular way.However,the applications of traditional Chinese medicinal herb are rare reported.As we all know,the invigorating spleen to remove phlegm is a very useful to reduce sputamentum which has been proved by long histories of traditional Chinese medicine(TCM),and it is believed that this method can aslo be used in the period of the postop.This research was designed to observe clinical symptom,sign,the white blood cell counting,and the incidence rate of post operative pulmonary atelectasisby a randomized,controlled,researchd design principle,therefore we can arrive a conclusion:whether the invigorating spleen to remove phlegm of TCM is useful to prevent pulmonary atelectasis or not.Method:Eighty patients who were underwent pulmonary lobectomy were randomly divided into two group:the experimental group and the control group,we observed postop 1^st,3^rd,5^th,7^thdays:①Clinical symptom:cough,phlegm quantity,expectoration degree,phlegm nature change;②sign:body temperature, breathing rate,pulse,blood pressure,rales;③Laboratory inspection:white blood count;④Sternum:pulmonary atelectasis or not.(1)experimental group were given fresh bamboo liquid 10ml,which inhalated by supersonic atomization way,bid;traditional Chinese decoction conluded by Four-noble drug decoction and Two-cure herb decotction,altogether 300ml,were divided 2 times to drink every day.(2)control group were given anbroxol hydrochloride injection 15mg and 0.9%physiological saline 10ml,which were inhalated by supersonic atomization way,bid;ambroxol hydrochloride tablets 30mg,bid.Result:There were no significant differences between the two groups of the following aspects:①total effectiveness,②body temperature,breathing rate, pulse,blood pressure③the white blood cell counting in the 1^st,3^rdday postop treatment,however,there were significance difference between the the two groups in white blood cell counting of the 5^thday,7^thday postop treatment (P＜0.05).There were not vicious reaction in the two groups,and no patients were let out because of medicine side effects,the experimental group is superior than the control group in the cost-effect contrast.Conclusions:This result indicates that the experimental group and the control group have the similar treatment effect.Invigorating spleen to remove phlegm method of TCM maybe another effective way to prevent pulmonary atelectasis caused by pulmonary lobectomy by thoracatory.This study is supportive to a larger application of the TCM and provides a new choice for the clinical application.Besides this,there are another advantages of Invigorating spleen to remove phlegm method of TCM,such as quick effects,less side effects, security,and no other extra pain and burden to the patients.this study is very promising and we will try our best to make further research.
