| Objective: By comparing such studies as biopsy, electron microscopy, immunofluorescence, immunohistochemistry in the healthy volunteers and patients with chronic tonsillitis, we attempted to explore the indexes which reflecting the pathogeny and grade of chronic tonsillitis. Based on the four diagnostic methods of TCM, the aim of this study was to determined the nature of different subtypes of chronic tonsillitis with histopathologyMethod: A total of 91 patients with chronic tonsillitis and 34 healty volunteers were selected from the Clifford Hospital of Guangzhou TCM University .We had collected specimens from bilateral tonsils before operation and then assessed through a series of biopsy, electron microscopy, immunofluorescence, immunohistochemistry and so on. The study data were systematicaly analysed by using the SPSS 13.0 statistical software package for Windows.Result: Compared with the healthy volunteers, bacterial culture of tonsil surface was not significant different in the patients with chronic tonsillitis(P >0.05), but germiculture of tissue biopsy was significant different(P <0.05). Signicant difference was also found among different symptomatic types(tanyuhujie,piweixuruo and feishenyinxu) (P < 0.05). Under electron microscopy,obviously abnormal keratosis of crypt epithelial cells and bacteria could be seen in the chronic tonsillitis group.On transmission electron microscopy, neutrophile granulocyte and non neutrophile granulocyte could be found among the crypt epithelial cells, even coccic bacteria could be encuntered in the crypt epithelial cells. The ratio of CD4+ T cells to CD8+ T cells was 3:1 in the control group and 1:1 in the chronic tonsillitis group, analyzing data from specimens of follicle and coranal. The proportion of CD8+ T cells in the tanyuhujie group was higher than that of piweixuruo and feishenyinxu groups (P < 0.05) . Compare to the control group, there were different degrees of keratosis in the chronic tonsillitis group. The results of cytokeratin was significant different among the three subtypes of chronic tonsillitis, attributing to longer course of diseases and higher grade in tanyuhujie group(P<0.05). Conclusion: Germiculture of tonsilar biopsy was a feasible method to investigate the pathogeny of chronic tonsillitis; electron microscopy corroboratesd the results of germiculture of biopsy, finding obviously abnormal keratosis of crypt epithelial cells and pathogenic bacteria; the ratio of CD4+ T cells to CD8+ T cells demonstrated cellular immunity playing an important role in developement of chronic tonsillitis; research of cytokeratin authenticated different degree of keratosis of chronic tonsillitis.Among the three different symptomatic types of chronic tonsillitis,the results of germiculture of tonsilar biopsy,electron microscopy,ratio of CD4+ T cells to CD8+ T cells and expression of cytokeratin were significantly different. |
