Objective Acute fatty liver of pregnancy (AFLP) is a emergent and serious entity associated with significant maternal and neonatal mortality and morbidity. We describe our experience with the clinical diagnosis, management and course patients with AFLP by retrospective analysis of patient with AFLP. Methods Medical records of 22 patients with AFLP were reviewed over a 8-year period. Records were reviewed for symptoms, laboratory findings, clinical course, and maternal and perinatal outcomes. Results The mean gestational age at onset was 38.6 weeks. 27.2% of patients were twins pregnancy. The prodromic phase was variable; patients complained of nausea, vomit, malaise and abdominal pain followed by jaundice. The laboratory results indicated WBC increased, coagulopathy, liver function abnormalities and hypoglycemia. Emergency cesarean section was performed in 22 cases, subtotal hysterectomy was performed in 7 patients, death was 8 cases, improve discharge was 2 cases and rehabilitation discharge was 12 cases. Of 28 fetus, stillbirth was 8 cases, neonatal death was 2 cases and survival was 18 cases, of which, sever asphyxia was 4 cases and light asphyxia was 12case.Maternal and fetal mortality is 36.3% and 35.7%, respectively. Conclusion AFLP is an emergent and serious complication of pregnancy. Regular prenatal check-up is help for early diagnosis, prompt delivery and intensive supportive care are the best approaches.
