BackgroundJinan City is the capital city of Shandong Province and its level of economic development is relatively high. The people's material living standards are improving. Lixia district is one of Jinan's urban districts, is also the Shandong Province provincial level institution, the universities and technical institutes colleges and universities most centralized area. The entire district population average culture quality and the living standard occupy the maximum level in Shandong Province and enjoys the healthcare service relatively is also good. With the economical swift growth, the industrialization and the urbanized advancement speeding up, the environmental pollution and behavior hazard factors had brought the huge influence to many disease's occurrences and development and caused the resident mortality rate and the cause of death constitution change. The national disease monitoring data statistics in 2000 pointed out that Cardiovascular diseases, Diseases of respiratory system and Neoplasms already became the primary cause which our country residents die. The study shows that in 2001 in Shandong Province the primary death causes of the surveillance population were Cardiovascular diseases, Neoplasms, Chronic respiratory diseases and Accidental injuries. Compared with 1991, the proportion of Neoplasms and Cardiovascular diseases increased by 6.03 percent and 2.64 percent. It is important to identify the death level, causes of death and trend of changes, as well as related risk factors, which is the foundation work in disease prevention and control. Lixia district is one of national disease surveillance points, collects the basic sanitation material chronically and continuously in the monitor crowd. But it has not yet been carried on that the system reorganization and the cause of death analysis as well as burden of diseases research, which based on death monitoring data from 1991 to 2005. To further use this information resource fully, we have the necessity to carry on a more thorough analysis on its 15 year cause of death statistical data. To analysis characteristic of death causes and burden of diseases from different side and angle can serve for health policy decision-making and disease control and prevention.ObjectiveIn order to find out the main death causes, the tendency of changes, regular pattern of death, health status and the factors influencing health status of the residents from 1991 to 2005 and to provide the scientific basis for the healthcare policy-making, hygienic plan formulation, health resources deployment.MethodsThe classification of death causes has taken ICD-9 as the standard from 1991 to 2003, and ICD-10 started to be used in 2004. In order to guarantee the material analysis the continuity and the concordance, this analyzes data still based on the ICD-9 code. Used Excel2003, SPSS11.5 and SAS9.0 carries on the descriptive statistical analysis. This study used age-specific proportion of the fifth national census in 2000 to carry on standardization to the data.1. Index of demography characteristic: including population size, proportion of sex, population age structure type, and population burden coefficient.2. Index of death level: including crude death rate, standardized mortality rate, and death rate and cause-specific death rate in different age and gender.3. Index of proportion and sequence of dying: proportion and sequence of death causes in different age and gender.4. The trend of death causes analyze5. life expectancy(ex=Tx / lx) and cause eliminated life expectancy (ex-i=Tx-i / lx-i)6. Index of burden of diseases: PYLL (years of potential life lost), WPYLL (working years of potential life lost), VPYLL (valued years of potential life lost) and FPYLL (family years of potential life lost). Results1. The proportion in 0-14 year old, 15-64 year old and 65 year old and above the group was 14.07%,77.37%,8.56%, whole families ratio was 60.84%, median age was 33.31 years old, Lixia district had already marched into the old age society. During 15 years the youth population burden coefficient present decreases the tendency, but the old population burden coefficient was in the rise.2. From 1991 to 2005 the standardized death rate in male was higher (451.51/105) than in female (419.48/105) (x2=16.676, P=0.000). Male the mortality rate which in the Neoplasms, Injury and poisoning as well as diseases of the digestive system was higher than the female obviously. The male life expectancy of 15 years (75.42 years old) and extending years (3.30 years old) were lower than the female (77.97 years old, 4.05 years old). The life difference between male and female expanded from 1.31 years old (1991) to 2.06 years old (2005).3. From 1991 to 2005, Diseases of the circulatory system was located at first and occupied 53.91%. SPYLL of Diseases of the circulatory system was 2338.20 man-years, occupied 38.46%, and was the first place of death causes which affected the resident health. After eliminating Diseases of the circulatory system, the resident life expectancy might enhance 15.71 years old, grew 20.37%.4. From 1991 to 2005, Neoplasms was located at two and occupied 23.89%. SPYLL of Neoplasms was 2338.20 man-years, occupied 38.46%, and was the first place of death causes which affected the labor force population health.5. Mortality rate of Endocrine, nutritional and metabolic diseases, and immunity disorders present the trend of rise.6. From 1991 to 2005, Injury and poisoning had been located at fourth in three times. Its AYLL, SVPYLL and SFPYLL were 32.84 years, 206.39 man-years and 1022.75 man-years. It was the first place of death causes which caused the resident life, the social value and the family to lose.7. Mortality rate and sequence of dying of Infectious and parasitic diseases dropped gradually and present the declining trend. ConclusionLixia district had already marched into the old age society. In future work men should be given more attention. The Noncommunicable disease harm was quite still prominent. Injury and poisoning was the primary cause which caused the resident life to lose. Mortality rate and sequence of dying of Infectious and parasitic diseases dropped gradually.Suggestions1. The related department should pay attention to the old age crowd highly the life state of health, raises their self-health care consciousness and ability, and thus improves old population's quality of life.2. The Department concerned should adopt the effective way to raise male self-health care consciousness.3. Suggested that the Department concerned while continued to enlarge prevents and controls Diseases of the circulatory system, takes Neoplasms and Injury and poisoning as the prevention key.
