Objective:To discuss a rapid,simple,applied and earlier diagnosis method of m ycoplasma pneumoniae in respiratory tract infection.To approve the advantage o f mycoplasma pneumoniae rapid liquid culture by comparing the clinical value in detecting mycoplasma pneumoniae of mycoplasma pneumoniae rapid liquid culture and cold agglutination test.To provide the laboratory evidence for clini cal selecting antibacterial drug.Method:MP rapid liquid culture was used to 150 cases with respiratory tract infection.150 cases with respiratory infection were tested by sputum and phary nx swab culture.And all were tested by cold agglutination test.There are 78 ma le and 72 female.From 3~94 years old,Including 25 cases under eighteen;67 c ases from eighteen to sixty;58 cases more than sixty.There are 142 cases low er respiratory tract infection,including 48 acute embitter of chronic obstructive pulmonary emphysema disease,34 pneumonia,21 lung cancer combinated with i nfection,20 bronchitis,11 bronchial asthma combinated with infection,4 brochie ctasis combinated with infection,4 pulmonary fibrosis combinated with infection. And there are 8 cases upper respiratory tract infection.Results:In the 150 cases with respiratory infection,54 cases were positive for rapid liquid culture assay and the positive rate was 36.0%(54/150).And 21 cases were positive for cold agglutination test and the positive rate was 14.0% (21/150),16 cases were positive for both(16/150).There were statisitical differenc es(x~2=25.3,P<0.05).we can arrival at a conclusion that MP rapid liquid cul ture in detecting mycoplasma pneumoniae precede to cold agglutination test.T here were no statisitical differences between male and female for MP rapid liq uid culture and cold agglutination test(x~2=0.43,P>0.05 and x~2=0.19,P>0.05). There were statisitical differences for vary age group for MP rapid liquid cult ure and cold agglutination test(x~2=15.2,P<0.05 and x~2=8.41,P<0.05)。There were no statisitical differences for under 18 year for MP rapid liquid culture a nd cold agglutination test(x~2=1.33,P>0.05).The positive rates for MP rapid liquid culture in detecting mycoplasma pneumoniae for 18~60 year is 37.3% (25/67),the positive rates for cold agglutination test in detecting mycoplasma pneumoniae for 18~60 year is 11.9%(8/67).There were statisitical differenc es for 18~60 year for MP rapid liquid culture and cold agglutination test(x~2 =10.24,P<0.05).he positive rates for MP rapid liquid culture in detecting mycoplasma pneumoniae for more than 60 year is 31.0%(18/58),the positiv e rates for cold agglutination test in detecting mycoplasma pneumoniae for 1 8~60 year is 8.6%(5/58).There were statisitical differences for more than 6 0 year for MP rapid liquid culture and cold agglutination test(x~2=9.39,P<0.0 5).we can arrival at a conclusion that MP rapid liquid culture in detecting myc oplasma pneumoniae precede to cold agglutination test for adult.In the 150 cases with respiratory infection,54 cases were positive for rapid l iquid culture assay.We got hold of 148 eligible sputum and isolated 59 indivi dual bacteria from 54 cases' sputum culture which were positive for rapid liqu id culture assay.There are 42 individual Gram-negative bacilli,Including 10 in dividual Acinetobacter,8 individual Pseudomonas,7 individual Enterobacter,6 in dividual Klebsiella,5 individual Escherichia,3 individual Stenotrophomonas,2 in dividual Citrobacter,1 individual Hafnia;There are 20 individual gram-positive c occi,Including 9 individual Staphylococcus,2 individual streptococcus;There are 6 individual fungi,all are albicans Saccharomyces.There are no gram-positive bacilli.54 cases were positive for rapid liquid culture assay.There were 30 cases whic h were no colony formation.We treated them with Azithromycin(A),β-lactam (B),Azithromycin withβ-lactam or Levofloxacin,moxifloxacin etal quinolones (C),period of treatment was 10~14 days.The group of Azithromycin including 7 cases,6 cases were in effect.The clinical efficiency rate was 85.7%(6/7).T he group ofβ-lactam including 8 cases,1 case was in effect.The clinical effici ency rate was 14.3%(1/8).The group of Azithromycin withβ-lactam or Levof loxacin,moxifloxacin etal quinolones including 15 cases,14 cases were in effec t and the clinical efficiency rate was 93.3%(14/15).There were statisitical diffe rences for vary project(x~2=17.30,P<0.05).24 cases were positive for rapi d liquid culture assay combinated with bacteria or fungi infection.We treated t hem with A,B,C,period of treatment was 10~14 days.The group of Azithrom ycin including 7 cases,2 cases were in effect and the clinical efficiency rate w as 28.6%(2/7).The group ofβ-lactam including 10 cases,6 cases were in effe ct and the clinical efficiency rate was 60.0%(6/10).The group of Azithromycin withβ-lactam or Levofloxacin,moxifloxacin etal quinolones including 7 case s,all cases were in effect and the clinical efficiency rate was 100.00%(7/7).Th ere were statisitical differences for vary project(x~2=7.66,P<0.05).Conclusion:MP rapid liquid culture is simple,rapid and no hurt.It can be use d for the early diagnosis of Mp infection.There is important value for the earl y diagnosis of mycoplasma pneumoniae.If the specimens including generous ba cterium which can decompose urea,the bacterium could influence the medium, and result false cultivation for mycoplasma.This research display that for case s which were positive for rapid liquid culture assay and the sputum culture w ere no colony formation,the effect which were treated by Azithromycin,Azith romycin withβ-lactam or Levofloxacin,moxifloxacin etal quinolones was batt er than only withβ-lactam.This conclusion clew that we should choose Azithr omycin,Azithromycin withβ-lactam or Levofloxacin,moxifloxacin etal quinolo nes for cases which respiratory infection to cover with MP.
