Objective:The purpose of this study is found the deficiencies of the work in the living donor liver transplant operation,so as to formulate rationalization,standardized and scientific processes of care.To discuss the validity and the feasibility of using the care processes in the living donor liver transplant operation.Promote the work of improving clinical care and seek effective methods of work,for clinical care for reference.Methods:This study is to select 70 patients of living donor liver transplant from 2007-2 to 2008-2 in Tianjin First Central Hospital.Will meet the requirements of the 70 cases of live donor liver transplant patients were randomly divided into intervenor and comparision groups,intervenor group:35 cases,comparision group:35 cases Application of living donor liver transplant care process for the intervenor,does not require application of living donor liver transplant care process for the comparsion group.Two groups of subjects on the operation time,the cost of surgery,surgery with satisfaction,the donor liver transplantation and cold ischemia time of receptors in the liver without advance,without a new liver and liver period of the blood gas(PH,BE,HCO3-,PaCO2,Na+,K+,Cl-,Ca+),hemodynamics(HR,MAP,CVP), and other indicators of change,statistical analysis,And the impact of living donor liver transplant operation the effect of factors related to a further study to clarify the processes of care and positive role.Result:1.The two groups in gender,age,education,illness and surgical cases,the difference was not significant(p>0.05),are comparable.2.The results showed that:living donor liver transplant care of the experimental group and the process does not require application of the care process control group in the operation time,cost of surgery,surgery with satisfaction,the donor liver cold ischemia time in a statistical difference Significance,P<0.05,that living donor liver transplant process is a well-established nursing care. 3.Application of living donor liver transplant care of the experimental group and the process does not require application of the care process in the control group of transplant recipients in a pre-liver,liver period with no new liver phase of blood gas(PH,BE,HCO3-,PaCO2,Na+,K+,Cl-,Ca+),hemodynamics(HR,MAP, CVP),and other indicators of changes in statistical group on February 2,with single-factor analysis of variance.New phase of the indicators of liver and liver without pre-and non-liver period compared with p<0.05,that implantation of the new liver are functioning.Conclusions:Application of living donor liver transplant care processes so that nurses participate in the operation of the work can be done in steps to clear link closely,in an orderly manner,an idea,rather than busy chaos,and ensure the smooth progress of surgery.Operating room nurses care process in accordance with the contents and methods of living donor liver transplant operation,care is no longer blindly and mechanically,such as the implementation of doctor's orders or instructions after doctors care for patients,but a planned and predictable,targeted,A more effective manner.The implementation of living donor liver transplant care processes so that the surgical care,orderly and effectively improved the nurses and coordination between the various professional capacity,and thus avoid personal level,capacity caused by the different work of the omission,neglect,error,raise A doctor of clinical nursing work satisfaction.A strict working procedures,time requirements,a planned care processes can be standardized care,improved medical care and the quality of service.
