Objective:To investigate the influence of Lai Fu Cheng Qi Decoction(LFCQD)on pulmonary function,serum Ghrelin and cytokine PAF after abdominal surgery and approach the therapeutic mechanism on gastrointestinal and lung function.Research the relationshio between Ghrelin & PAF and lung function.Methods:36 case were selected and divided into two groups randomly,treated with Lai Fu Cheng Qi Decoction enema from 1 to 3 day of postoperation and routine treatment respectively(control group).Forced vital capacity(FVC),forced expiratory capacity in first second(FEV1.0),maximal voluntary ventilation(MVV),arterial O2 pressure(PaO2)and arterial CO2 pressure(PaCO2)were measured by lung function equipment and blood gas analysis equipment pre and post operation day 1,3, 5.Another 36 case were selected and divided into two groups on the basis of modus operandi:hepatobiliary surgery and gastrointestinal surgery,18 in each group.Every group were divided into two groups randomly,treated with Lai Fu Cheng Qi Decoction enema from 1 to 3 day of postoperation and routine treatment respectively(control group).The level of serum platelet activating factor(PAF),Ghrelin,FVC,PaO2 and exhaust defecation time.Results:1.The level of FVC,FEV1.0,MVV and PaO2 decreased significantly (P<0.05)in Chinese herbs group and control group on postoperative day 1 and increased on postoperative day 3.The change on the third day in Chinese herbs group was more significant than controls.FVC,FEV1.0 and MVV increased more significantly in Chinese herbs group than controls on postoperative day 5(P<0.05).The difference of PaO2 between Chinese herbs grup and control group was not significant.The alteration of PaCO2 in Chinese herbs group was slight but nonsignificant.The level of PaCO2 increased significantly(P<0.05)on postoperative day 1 in control group. 2.The level of PAF in hepatobiliary surgery and gastrointestinal surgery group increased significantly(P<0.05),deceased post-operation day 3. There was a statistical significance difference between Chinese herbs group and controls(P<0.05).The level of PAF on the third postoperative day and the fifth day in Chinese herbs group decreased more significantly than controls(P<0.05).The level of serum PAF is negatively correlated with the level of FVC and PaO2.3.The level of Ghrelin in hepatobiliary surgery and gastrointestinal surgery group increased significantly(P<0.05),deceased post-operation day 3. The level of Ghrelin of patients undergoing hepatobiliary surgery in Chinese herbs group decreased more significantly than control on the third and fifth postoperative day(P<0.05).There was a statistical significance difference between the level of Ghrelin of patients undergoing gastrointestinal surgery in Chinese herbs and control group on the third postoperative day and on the first postoperative day(P<0.01). The level of Ghrelin decreased more significantly in Chinese than controls(P<0.05).The level of serum Ghrelin is negatively correlated with the level of FVC and PaO2(P<0.01).4.The recovery of exhaust defecation time in Chinese herbs group was earlier than control group.Conclusion:1.Lung function decreased after abdominal selective surgery.The recovery of FVC,FEV1.0,MVV and PaO2 by using Lai Fu Cheng Qi Decoction was better than control group.The study showed th at lung function was injured by abdominal operations.Lai Fu Cheng Qi Decoction promote pneumonic function recovery,diminish lung injury after surgery.2.The relationship between Ghrelin,PAF and lung function was closed.3.Lai Fu Cheng Qi Decoction could decrease the level of PAF and regulate the level of Ghrelin.The drug promoted enteric function recovery,decreased the release of inflammatory mediator and diminish lung injury after abdominal surgery.
