Objective:To investigate the feasibility and the significance of radio-guided.Methods:14 patients undergoing curative resection of rimary colorectal chancer with extended lymph node dissection were included in this study.99Tc-sulfur needle(patients with rectal cancer were injected throuth anus before operation two minutes).Mpping lymphatic wer methyltionius stain.Ten ninutes later,hot spots were identified by lymphoscintigraphy with SPECT.andγprobe-directed was used intraoperatively to locate SLNs reached 10 times more radionactivity than surrounding tissue.The specimens wer analyzed in the usual fashion.Nodes with high radionactivity that were negative by hematoxylin and eosin(HE)staining wer further examined by immunostainingResult:γprobe-directed SLNs wer identified in 13 of 14 patidents.45 SLNs were detected with a mean number of 3.2 nodes per case,ranged from 2 to 4.Most(75.6%)of SLNs were found in N₁ area,but 1 case were in the N2 area without N1 involvement.The incidence of metastasis was significantly higher in SLN than in non-SLN(62.2%VS19.6%).The accuracy of the SLN metastatic status in diagnosis of the regional lumph node status was 92.3%.The false-negative rate was 7.7%.With immunohistochemical staining.lymphatic pathologic staging in 1/14patients was upstaged.γprobe-directed sentinel lymph node and Identificationof Sentinel Lymph Node with Methlene Blue delection for colorectal cancer is the same.Conclusion:1 SLN concept is feasible for colorec tal cancer;radio-guided andγprobe-directed SLN detection is an accrate diagnostic procedure for detecting lymph node metastasis in patients with colorectal cancer.The incidence of metastasis was significantly higher in SLN than in non-SLN.2 SLNs were found in N₁,N₂ areaγprobe-directed sentinel lymph node and Identificationof Sentinel Lymph Node with Methlene Blue delection for colorectal cancer is the same...
