Study of the correlation between plasma leptin and symptom and antipsychotic therapy of schizophrenic patientsThe plasma leptin is a lipide hormone,which mainly assists lipide metabolism,accommodates diet and weight gain.However,more function was found with the development of the study about it.It not only impacts on body gain by psychotropic drug,but also takes a important role in thinking,affection and behavior of schizophrenia.This study proposes to authenticate such hypothesis.一,plasma leptin and impulse,suicidal behavior of schizophrenic patientsObjective To investigate the correlation between impulse,suicidal behavior and plasma leptin and total cholesterol of first-episode schizophrenic patients.Method Selected 106 first-episode schizophrenic patients assessed by Brief Psychiatric Rating Scale(BPRS),Positive and Negative Syndrome Scale(PANSS),Hamilton Depression Scale(HAMD-24) and Beck Holepessness Scale(BHS),were organized into 3 groups:suicide group(n=24),impulse group(n=31) and non-suicide and non-impulse group(n=51).In addition,32 individuals who took part in health examination were selected as health control group.Body height and weight were measured to get body mass index(BMI) of all objects,and plasma leptin and total cholesterol were measured by venous blood before taking pill.Result①There was no significant difference in ageing,sex,case history,BMI,and BMI between male and female in every group by analysis of variance(P<0.05).②As for total cholesterol,health control group and non-suicide and non-impulse group was obviously higher than suicide group and impulse group(P<0.05,0.01 and 0.05,0.05,respectively),however,there was no significant difference between suicide group and impulse group(p>0.05),and between health control group and non-suicide and non-impulse group(p>0.05).As for plasma leptin,health control group was higher than non-suicide and non-impulse group,suicide group and impulse group(P<0.05,0.01 and 0.01,respectively), non-suicide and non-impulse group higher than suicide group and impulse group(P<0.05,respectively), but there was no significant difference between suicide group and impulse group(p>0.05).③Plasma leptin and total cholesterol in the four group were positively correlated with BMI(all p<0.05 or<0.01); plasma leptin and total cholesterol in suicide group and impulse group were inversely positively correlated with PANSS positive score,PANSS attack score,BPRS score,HAMD score and BHS score, moreover,plasma leptin was significantly positively correlated with total cholesterol between the two groups(γ=0.61,0.58,all p<0.01);plasma leptin in non-suicide and non-impulse group was inversely negatively correlated with PANSS positive score,PANSS attack score,BPRS score,HAMD score and BHS score,while total cholesterol was significantly negatively correlated with PANSS attack score and BHS score(γ=-0.48,-0.35,all p<0.05),and had no correlation with PANSS positive score,PANSS negative score,BPRS score and HAMD score(p>0.05).Conclusion Plasma leptin of first-episode schizophrenic patients was lower than that of health control group,moreover,and it was the lowest of patients with impulse and suicidal behavior.Plasma leptin of schizophrenic patients with impulse and suicidal behavior was significantly positively correlated with total cholesterol,and negatively correlated with impulse and suicidal behavior.二,plasma leptin and antipsychotic drugAIM:To explore the effects of clozapine,risperidone,olanzapine,quetiapine and haloperidol on plasma leptin in first episode schizophrenic patients.METHODS:114 first episode schizophrenics were randomly divided into five groups,which were treated with clozapine,olanzapine,quetiapine, risperidone and haloperidol,respectively.The plasma leptin concentration was measured with radioimmunoassay pre treatment and 6 weeks post treatment.RESULTS:①Plasma ieptin level of post-treatment[(12.88±3.12)μg/L,(11.84±4.26)μg/L,(11.52±3.44)μg/L,(11.38±2.92)μg/L, respectively]increased compared with that of pre-treatment[(9.27±2.67)μg/L,(9.47±3.24)μg/L,(8.70±1.49)μg/L,(10.51±2.91)μg/L,respectively]in both clozapine group(P<0.01),risperidone group(P<0.05),olanzapine group(P<0.05) and quetiapine group(P<0.05),and significant difference of the increased value of plasma leptin was found after treatment between clozapine,olanzapine and haloperidol groups(P<0.05).②Body mass[(25.40±3.41) kg/m2,(25.27±4.03) kg/m2,(25.93±4.72) kg/m2,(24.99±2.97) kg/m2,respectively]after treatment increased compared with that before treatment[(23.11±3.45) kg/m2,(24.47±2.97) kg/m2,(23.88±3.09) kg/m2,(24.31±3.29)kg/m2,respectively]in both clozapine group(P<0.01),risperidone group(P<0.05), olanzapine group(P<0.01) and quetiapine group(P<0.05),and significant difference of the body mass gain was found between clozapine,olanzapine group,risperidone,quetiapine group,and haloperidol groups respectively(P<0.05).③The increased values of plasma leptin at the 6th weekend were positively correlated to the body mass gain in both clozapine group(r = 0.714,P<0.01),risperidone group(r = 0.625,P<0.01),olanzapine group(r = 0.594,P<0.01) and serquel group(r = 0.735,P<0.01).CONCLUSION:Both clozapine,risperidone,olanzapine and quetiapine have significant effects on plasma ieptin concentration and body mass,and clozapine and olanzapine has stronger effect than risperidone and quetiapine on body mass,while haloperidoi has no obvious effect on plasma leptin and body mass.三,plasma leptin and the weight gain induieed by antipsychotic drugObjective:To explore the effects of anti-psychiatric drugs on patients' body weight gain and its correlation factors。Methods:78 episode schizophrenics were recruited。The body weigh,plasma leptin,insulin,insulin antibody,fasting plasma glucose,glycosylated hemoglobin,cholesterel and TG concentration were measured pre-treatment and 6-week post-treatment,Results:①The body mass of pre-treatment(61.03±8.74kg) has significent difference compared with the body mass of post-treatment(63.77±9.56kg)(P=0.000),②The body mass gain were negatively correlated to TG of pre-treatment(γ=-0.400,P<0.01),fasting plasma glucose level of pre-treatment(γ=-0.246,P<0.05) and the cholesteral level of post-treatment(γ=-0.330,P<0.01).but the body mass gain were positively correlated to insulin level of pre-treatment(γ=0.231,P<0.05),insulin level of post-treatment(γ=0.234,P<0.05) and the change of leptin level(γ=0.269,P<0.05)。③The results of multiple linear regression analysis showed that the major influencing factors of weight gain were the kinds of anti-psychiatric drugs,TG level of pre-trenament and the changes of leptin level。Conelusion:The patients have significent weight gain after anti-psychiatric treatment。The major influencing factors were the kinds of anti-psychiatric drugs,TG of pre-treatment and the change of leptin level。...
