Depression And Its Influening Factors In Chronic Hepatitis Patients

Objective: To research the depressive disorder situation and its influencing factors of the chronic hepatitis patients, in order to get new methods to treat the depressive disorder and promote the mental health level for the chronic hepatitis patients.Methods: A questionnaire survey was carried out among the chronic viral hepatitis patients (clinically diagnosed and chosen in an infectious hospital) by means of random Stratified cluster sampling method. The questionnaire included Eysenck Personality Questionnaire, self-rating depression scale (SDS), and the self-designed ones regarding general conditions, disease information and social support. We distributed 230 questionnaires, and reclaimed 223, and among the 223 questionnaires there were 2 whose coarse disguise points (L scores) > 18 and 9 whose information was not complete were removed, so a total of 212 patients containing 132 patients with HBV and 80 patients with HCV were chosen as our subjects( the patients whose SDS≥0.5 were classified into the case group, and the others were considered as the control group ) . All the data we collected was saved and analyzed with SPSS 13.0 software.Result:(1)Depression incidence rate of chronic viral hepatitis patients was 54.7% (116/212), including mild 51.7% (60/116), moderate 42.2% (49/116) and severe 6% (7/116); the incidence rate of depression between CHB group and CHC group wasn't different significantly (χ~2=1.154, P>0.05), and no significant difference was found between in-patients and out-patients(χ~2=0.519, P>0.05) .(2)These factors such as age, occupation, education, economic situation, methods of payment, indexing viral hepatitis infection, confirmed time, the number of recurrent hepatitis, anti-virus treatment time, confidence for recovering, satisfaction with around may be associated with depression, and the severity of the depression was inverse correlative with the social support score, score of objective support , score of subjective support and E-scales on the Eysenck Personality Questionnaire, positive correlative with the score of motion stability.The severity of depression (depression index) was significantly negative correlated with social support scores, objective support scores subjective support scores (r =- 0.276, P<0.001 and r = -0.224, P = 0.001 and r =- 0.284, P<0.001) ,and there was correlation between severity of the depressive disorder and Eysenck Personality two dimensions sores, scores of introversion and extroversion scores negatively correlated (r =- 0.333, P<0.001 )and emotional stability scores positive correlated (r = 0.269, P<0.001). (3) The results of case -control study : 14 of 31 factors were with statistical significance, and 11 of the 14 factors were risk factors (OR>1), including occupation, education, economic situation, infection time, indexing viral hepatitis infection, confirmed time, emotion stability, the number of recurrent hepatitis, confidence for recovering, satisfaction with around and E-scales on the Eysenck Personality Questionnaire, and the other 3 were protective factors(OR<1), including weight, body height and worrying about the infection.The mean weight of the case group was higher than control group(t'=2.114, P<0.05).The social support scores, scores of objective support and scores of subjective support of the experiment group were lower than the control group(t'=-3.79, P<0.001; t'=-2.27,P<0.05; t'=-3.84,P<0.001)The case group's scores for the P-scales and N-scales on the Eysenck Personality Questionnaire were higher than that of the normal control group (P<0.01), but scores for E-scales was lower (P < 0.001).Logist regression analisis results: Hepatitis Indexing(OR=2.799, OR95%CI:1.405-5.576), infection time(OR=1.566,OR95%CI: 1.202 - 2.041), emotional stability(OR=2.713, OR95%CI: 1.690-4.357), satisfaction with around (OR=2.127, OR95%CI: 1.163-3.889), occupation(OR=2.076, OR95%CI:1.389-3.102), and education(OR=1.636, OR95%CI: 1.044-2.564) were found to be the influential factors of depression in chronic viral hepatitis patients.Conclusion:(1)Morbidity of depression in patients with chronic viral hepatitis is high.(2)These factors such as age, occupation, education, economic situation, methods of payment, indexing viral hepatitis infection, confirmed time, the number of recurrent hepatitis, anti-virus treatment time, confidence for recovering, satisfaction with around may be associated with depression, and the severity of the depression was inverse correlation with the social support score.(3)Hepatitis severity, the infection time, patients' personality characteristics, social factors may be the main factors related with the depression of chronic viral hepatitis patients.
