Objective:Due to the irregular and asymmetrical right ventricle,it either has a substantial error to measure right heart function by using normal ultrasonic way,or can't be put into wide use mostly for technological or economical reasons by using the invadable or specific ultrasonic ways,such as transeasophageal three-dimensional echocardiography.There is still not a standarded method that can accurately and admittedly evaluate right ventricular function in clinic,The research is aimed to explore the clinical value of ameliorate Simpson method of transthoracic two-dimensional echocardiography(RVAA)in measurement of fight ventricular function in patients with essential hypertension.At the same time,probe chang of lung function in patients with essential hypertension and correlation between RVEF obtained from RVAA and lung function indexs.Method:1.Measurement of right heart function:60 patients with essential hypertension and 20 cases control group were checked in apical four chamber view on 2DE.Total ventricular volume(Vvl)(include interventricular septum)and Vlvivs (volume of interventricular septum and left ventricular)were measured and calculated by RVAA method,then got right ventricular volume(RVV= Vvl--Vlvivs).Ventricular volume measured by mensioned method in end systole and end diastole which can calculated right heart function indexs and compared them with result of single area-length method.2.repeated study on RVAA.3.All the examinees were checked their lung function,and the results obtained were analyzed comparatively with the RVEF by using RVAA.Result:1.There was no significant difference on right heart function indexs obtained respectively by RVAA and single area-length method in each group.2.It were found that RVESV in stageⅡwas higher than that of stageⅠ,RVCO in stageⅡwere lower than that of stageⅢ(P<0.05 or P<0.01)which could not be founded by using single area-length method.3,repeated study showed that coefficient of variation within group was2.69%and between group was 2.42%.4.lung function indexs in each EH group were lower than that of control group,but there were difference in FVC, FEV1,MMEF on stageⅡand all indexs on stageⅢ.compared with stageⅠ,FVC,FEV1 was decreased in stageⅡ,FVC,FEV1,MMEF were decreased in stageⅢ(P<0.05 or P<0.01);FVC in stageⅢwas lower than that of stageⅡ(P<0.01).5. Relative coefficient in RVEF from RVAA and FVC,FEV1,MMEF,MVV,TLC was respectively 0.93,0.91,0.59,0.51,0.28;there were main positive correlation within RVEF andFVC,FEV1(P<0.01).Conclusion:1.Compared with single area-length method,RVAA could better estimate right ventricular systolic function in patients with essential hypertension with easy,sensitive and repeated feature and it has clinical value.2.RVEF in patients with essential hypertension in stageⅢwas decreased.3.There was damaged of lung ventilation function in patients with essential hypertension which aggravated with increase of blood pressure level.3 There might be correlation between RVEF and lung ventilation function in patients with essential hypertension.
