Objective To research the principle of Anterior lobe of transverse mesocolon or Capsula pancreatis metastasis, to clarify the necessary and method of dissection in gastric cancer.Methods This study retrospectively investigated 45 patients undergone curative radical dissection for gastric cancer from March to October in 2007 in the 1st affiliated hospital of GuangXi Medical University, analyzed the clinicopathological factors which influenced the Anterior lobe of transverse mesocolon or Capsula pancreatis metastasis, including sex, age, location of the primary tumor, tumor sizes, Borrmann's type, invading depth, microscopic classification, lymph nodes metastasis and TNM staging after operation. The data was analyzed by SPSS13.0 softwares. P<0.05 was considered statistically significant.Results Among the 45 patients there were 26 males and 19 females. Their ages range between 24~77 years, the mean age of patients was 53 years. 4 patients were early gastric cancer(8.9%). 41 patients were progression gastric cancer(91.1%). Metastasis to lymph node happen in 33 patients(73.3%). Metastasis to Anterior lobe of transverse mesocolon or Capsula pancreatis happened in 17 patients (37.8%). statistics analysis showed location of the primary tumor, the depth of tumor invasion, microscopic classification and TNM staging were predictive factors for metastasis to Anterior lobe of transverse mesocolon or Capsula pancreatis.Conclusion 1. Because of the high metastasis rate, especially in the upper two third tumor, the depth of tumor invasion extends beyond the serosa and TNM staging III,IV. it is absolutely necessary to remove Anterior lobe of transverse mesocolon and Capsula pancreatis in gastric cancer for prevention or treatment. 2. It can remain the Anterior lobe of transverse mesocolon and Capsula pancreatis in the tumor invasion not to the serosa and the TNM staging I,II of gastric cancer in order to avoid the complication and mortality post operation.
