Study On Pre-S1 Antigen In Relation To HBV Replication And It's Change During Anti-virus Treatment

Objective:To study pre-S1 antigen in relation to hepatitis B virus(HBV)replication and it's change during anti-virus treatment with nucleoside analogue.Patients and methods:One hundred sixteen outpatients and inpatients with chronic hepatitis B in our hospital from June 2005 to February 2008 were enrolled,including 60 HBeAg positive patients and 56 HBeAg negative ones.Sixty two of these patients received lamivudine treatment,while the other 54 ones just received liver protection treatment.HBV DNA,HBV markers,pre-S1 antigen and liver function was examined separately at 0,24 and 48 week of treatment.ELISA and Fluorescence PCR was used in analysis ofpre-S1 and HBV DNA respectively.Results:The positive rate of pre-S1 antigen was separately 84.41%and 51.35%in patients with higher HBV DNA level(＞10~5copies/ml)and lower HBV DNA level (≤10~5copies/ml).There was significance(p＜0.05)between this two group.The positive rate of pre-S1 antigen was separately 83.33%and 64.23%in patients with HBeAg-positive and HBeAg-negative.There was significance(p＜0.05)between this two group.During lamivudine therapy,the positive rate of pre-S1 antigen between treatment group and control was no significant(8.33%vs.2.63%,p＞0.05)at 24 week, and significant(22.92%vs.5.26%,p＜0.05)at 48 week.90.9%patients with pre-S1 antigen loss were HBV DNA negative,whereas 34.48%patients with HBV DNA loss were pre-S1 antigen negative. Conclusion:The pre-S1 antigen has a good relation to HBV DNA level,and can regard as one of good index of lamivudine curative effect.It can act as a guide of diagnose and ant-viral therapy to chronic hepatitis B,particularly to HBeAg negative hepatitis B.
