Treatment For Acetabular Fractures And Its Complicatioans

Objective Analyzing the clinical experiences and the method for management in order to discuss the factors affecting the treatment results of acetabular fractures.Methods It was to summarize the experience in treatment of 53 cases of acetabular fractures treated operatively from 2002 to 2006 of which all fractures were classified according to the Letournel and Judet classification,the various surgical exposure accoding to the different types,and fixation were carried out with special instruments,pelvic reconstructive plates and screws.Results 26 were followed up for average 40.5 months(from 4 months to 60 months).Based on the modified Merle d'Aubigne and Postel clinical grading system,2 cases showed excellent results,15 good,5 fair and 4 poor.with an excelence rate of 65.4%. According to criteria of Matta radiographic grade,in the 51 cases after operation,there were 27 cases with anatomic reduction , 20 with satisfactory reduction , 4 with unsatisfactory .No death or infection occurred in the group.Avascular necrosis of the femoral head was found in 2 cases and traumatic arthritis in 12 cases.Conclusion Correct analysis of the displacement of acetabular fractures and the types,reasonable surgical approaches,timely surgery and anatomic reduction,surgeon's experience and active treatment of complications are essential to good clinical results.
