| OBJECTIVE: Studies have shown that the T-shaped plate can be pre-bent to a suitable angle and shape to block the fracture fragments in the quadrilateral region of the acetabulum,especially the fracture line extending to the greater sciatic notch.To investigate the effect of T-shaped plate in the operation of acetabular quadrilateral fracture.METHODS:A total of 31 patients who underwent surgical treatment for pelvic and acetabular fractures at the Second Affiliated Hospital of Inner Mongolia Medical University from January 2018 to April 2021 were selected as the experimental group.Among them,18 patients were treated with a combination of reconstruction plates and T-shaped plates to fix the fracture blocks in the quadrilateral area of the acetabulum.During the surgery,the reconstruction plates and T-shaped plates were pre bent according to the fracture situation,and the transverse plate on the head side of the T-shaped plate was bent towards the tail end,making the entire plate close to a right angle,Fix the fracture block at the top of the square area to match the fracture site;The control group consisted of 13 patients who were treated with pre bent reconstruction plates to fix the quadrilateral acetabular fracture blocks,while the other procedures were the same as those in the experimental group.After treatment,observe the patient’s surgical operation time,intraoperative apparent bleeding volume,surgical incision length,and fracture healing status;Evaluate the X-ray score and hip joint function of the affected side after fracture reduction.Results:(1)After 24-40 weeks of follow-up,the four-quadrant fracture was well reduced during the operation,the test group was placed with pre-bent reconstruction plate and T-shaped plate,and the plate contacted well with the bone surface;(2)The operation time of the test group was longer than that of the control group(P<0.05),and the intraoperative bleeding was less than that of the control group(P<0.05);(3)Fracture reduction was evaluated according to Matta standard,the excellent and good rate of the test group was 89%(16/18),and the excellent and good rate of the control group was 85%(11/13).The time of complete weight-bearing in the test group(7.9 ± 1.6)weeks was better than that in the control group(9.5 ± 2.1)weeks(P<0.05).Conclusion: The use of a T-shaped plate combined with a reconstruction plate for the treatment of fractures involving the quadrilateral area of the acetabulum has good reduction effect,simple operation,minimal intraoperative bleeding,and short complete weight-bearing time. |
