Objective: To investigate the expression and distribution of Il-6 in periodontal tissue of rats during orthodontic tooth movement and under the condition of inflammation, and to study the combined mechanism of inflammation and orthodontic force on the periodontal tissue remodeling in rats. Methods: 60 SD rats were randomly divided into 3 groups. 50 gram orthodontic force was loaded on the left maxillary first molars of 30 rats (group 1) whereas established experimental periodontitis to the other 30 rats. After 2 weeks experimental periodontitis was established, orthodontic force was loaded on the left maxillary first molars to be group 3, and the contralateral fist molars of these 30 rats was group 2. Immunohistochemistry and histomorphometric analysis was performed to measure the expression of IL-6 and the loss of alveolar bone at0,1,3, 5,7, and 10 days after the application of orthodontic force. .Results: The expression of IL-6 was observed to reach a maximum on day 3 and to decline thereafter in group 1 whereas the expression level of IL-6 in group3 increased compared to group 1 and reached the maximum on day 5 then declined slowly. The expression of IL-6 in group 2 can also be observed and declined rapidly after the local inflammatory stimulus was eliminated. Alveolar bone loss was detected in group 2 and group 3 but can not be detected in group1.Conclusion: Orthodontic force as well as inflammatory stimulus can evoke the expression of IL-6. Orthodontic force can not cause alveolar bone loss as long as the local inflammatory stimulus was eliminated. Orthodontic patient with gingivitis or periondontitis can be treated when periodontal inflammation was controlled by eliminating local inflammatory stimulus.
