Objective To discuss the effect of limited decompression and internal fixation on treatment for degenerative lumbar spinal stenosis. Methods 135 cases of degenerative lumbar spinal stenosis underwent operation were investigated retrospectively. 57 males,78 females aging from 36~80years (with the mean age of 61.3years),with case history of 5 months~20 years(averaging 16 months)were collected. The average age was 61.3year.According to Hansraj's classification of lumbar spinal stenosis, 52 cases were treated with limited decompression.27 cases were treated with limited decompression and internal fixation with bone graft fusion. 56 cases were treated with decompression and internal fixation with bone graft fusion. The clinical outcome was analyzed with a scoring system proposed by Japanese Orthopaedic Association(JOA) score and Eule assessment. Results With a mean follow-up period of 3.4years, the rate of the improved JOA score(RIS) were 58.2±34%,61.7±23.6% and 56.4±26.8%,shows no statistical difference among three groups. Patients were satisfied with surgical outcomes except 2 patient both in group A and C. Conclusion Strict indication and selection of cases are the key for success. we should emphasize limited decompression of operation. Both limited decompression alone and internal fixation all can obtain good result. posterior structure of vertebrae should be reserved by limited decompression to get better outcome.
