| Objective To investigate the safety, efficacy,and feasibility of laparoscopic incisional hernia repair (LIHR).Methods The outcomes for 58 patients who underwent incisional hernia repair in our hospital from January 2005 to October 2007 were analyzed retrospectively.The outcomes for 12 patients who underwent laparoscopic incisional hernia repair (LIHR) were compared with those for 46 patients who underwent open repair (OIHR) during the same period. Results There was no significant difference between two groups in gender, age, general hernia characteristics etc(P>0.05). The mean operative time was shorter in LIHR group ( 81.67 vs 120.52 minutes ; P〈0.01). The mean blood loss in LIHR group was 15.83ml less than 73.80 ml in OIHR group;(P〈0.01). The mean postoperative bowel function recovery time was shorter in LIHR group ( 1.25 day vs 1.85day ; P〈0.01 ). The postoperative analgesic requirement was lesser ; (P〈0.05)and the hospital stay was shorter (2.7vs 4.7 days; p < 0.05) in the LIHR group. Complications occurred in 24% of the laparoscopic and 30% of open group. ( p = 0.538, not statistically significant). The cost of overall hospital stay was lesser for laparoscopic procedure, ( p = 0.734, not statistically significant).Of the 12 patients in LIHR group 11patients followed for a mean of 15 months (range, 3～27 months) no recurrent case were noted ; Of the 46 patients in OIHR group 35 patients followed for a mean of 17 months (range, 3～32 months) Recurrence occurred in one only case ,recurrence rate was2.9%.Conclusion The findings demonstrate that laparoscopic incisional hernia repair in our experience was safe and resulted in shorter operative time, fewer complications, shorter hospital stays, and less recurrence. Hence, it should be considered as the procedure of choice for incisional hernia repair.
