Heart failure (HF) is a kind of serious clinical complication. It is the end stage of several primary cardiovascular diseases. Due to its high incidence rate, low survival rate and cure rate, heart failure is becoming a public health problem all over the world. One pivotal problem in clinic is the lack of individual treatment decision strategy and effective method to evaluate the status of heart failure patients.Guided by medical knowledge, we applied computer modelling and system identification technology, in particular Set Membership Identification (SMI) algorithm, to establish an optimum therapy model for heart failure. The model is utilized to determine medicine therapy, and evaluate treatment efficiency. Accordingly, the following tasks were accompolished:1. Establishement of the database management system for heart failure patients;2. Establishement of the knowledge base for HF treatment, according to "ACC/AHA Guidlines for the Evaluation and Management of Chronic Heart Failure in the Adult";3. Visualization of the computerized and structured, clinical pathway for HF;4. Development of the optimum treatment strategy model based on Set Membership Identification;5. Realization of the model of patient therapeutic status.For the optimum therapy model, the inputs are physical examination data and the output is the recommending index of the chosen medicine treatment. The suggested decision is expressed in the form of optimal bounding ellipsoid solution set. The results show that, different therapy patterns have corresponding ellipsoid solution sets with specific location and size information in 3D space.The developed model is expected to provide a useful tool for curative effect evaluation and optimal therapy strategy of heart failure. This study offeres effective technical solutions for the convenient operation of clinic guideline, individual therapy decision and patient status evaluation.
