ForewordBreast cancer is endangering the health of women,it has received extensive attention.With the rapid development of economy in the past 20 years,the living standards of citizens changed constantly,the living habits of majority of women trend to western,with the incidence of breast cancer increased significantly,it has leapt to the female malignancyin the first place.Today,numerous studies have shown that epidemiological methods can be used on BC analysis of the relevant factors to guide women through self-assessment and health care to reduce the occurrence of breast cancer.All the current epidemiological investigation about the risk and protective factors of breast cancer are based on abroad and south area of china,there aren't samples of a large number of investigations and studies to be reported in our country's northern.Therefore,the case-control study by the research methods,menstrual history analysis,the certification has been reported in the literature and the female breast cancer incidence in the region related to the above-mentioned factors are consistent,whether the different characteristics of the region.Subjects and methods1.The selection of subjectsCollecting the patients with BC in the first hospital of China Medical University from 2001 to 2006 and the new cases and past cases in this census(have a clear diagnosis of breast cancer),after removing sub-standard,which will eventually as the remaining 596 as cases group(mean age =52.13±0.52years). BC census.Choicing female volunteer who are permanent residence in the 10 community offices to be had jurisdiction over two streets of Shenyang area,after removing sub-standard,which will eventually remaining 2,822 cases as a control group(mean age =47.64±0.29 years).2.The content of epidemiological surveyGatered the object of study and filling in the forms designed accordancing to epidemiological data,selected name,age,age at menarche and amenorrhea,the first living child age and family history of BC,the time limit of the menarche and the duration of the first living births to the menarche as research content.3.Diagnostic criteria(1)The case group are confirmed from pathological diagnosis of BC patients.(2)The control group are examed by physical examination,ultrasound and mammography examination,which confirmed that they are non-or benign lesions,or one of those suspected malignant,as confirmed lesions by pathology after in the census.4.The collation of information and statistical methodsA database was created by Excel after the quantity examination of data.The statistical methods were t-test,variance analysis,χ~2 test.P<0.05 considered statistically significant difference.The software used was SAS8.0.Results1.The distribution of age between case group and the the control groupThe average age of the case group is about 48 years old,the control group is about 52,the age distribution of the difference between the two groups(P<0.01)in the calculation need to be make adjustments.2.Composition of the family history of BC in the case group and the control groupCase group and the control group between a BC family history showed no significant difference(P>0.05).For they are both in the smaller family history,the composition of the two groups can be considered similar than different,they are comparability.3.Menstrual history of breast cancer associated with the single factor analysisTo compare age at menarche,age at menopause,age at the time limit of the menstruation and the duration of the first living births to the menarche in the different groups with a single logistic regression analysis,we found that the differences between the two groups were significantly(P<0.01)about the age at menarche and the duration of the first living births to the menarche and length of menstruation.Age at menopause had a weak effect to BC,but it hasn't significant in statistics(P>0.05).In the different age groups the result are slightly different.(1)Menarche age and BC-related single factor analysis after divisionThe premenopause crowd divised by≤35 year old,and>35 years old,with postmenopause crowd we found:the case and the control group were significant difference(P<0.01)in>35 years group.(2)The results of duration of the first living births to the menarche and BC after division seemed the case and the control group were significant difference(P<0.01) in>35 years group too.DiscussThere have sufficient evidence that come from the experimental,clinical and epidemiological studies show that:the hormone plays an important role in the occurrence of BC,and the view that reproductive factors,including menstruation can be seen as the estimation of the breast cumulative estrogen exposure.The study found that early age at menarche and the long-interval duration of the first living births to the menarche are risk factors to BC,and they are consistent with those reported in the literature in past.Age at menopause had a weak effect to BC,but it hasn't significant in statistics(P>0.05).length of menstruation had a negative correlation to BC.From this study the results can be seen in the early age at menarche is a risk factor of BC,and will reduce the risk with age growth.After stratification,in the age≤35 years premenopausal group and poetmenopause there are not significantly different(P>0.05)with BC between the case and the control group.In the>35 year-old premenopausal women,early age at menarche the incidence of BC is important,the danger diminishes with the increase in age at menarche.The standard of the impact that the duration of the first living births to the menarche with BC on the domestic and foreign literature are reported rare.The study found that in the case and the control group there is significant differences(P<0.01), OR value increases with the increase of duration.ConclusionThis study indicates that:the relationship between the menstrual history and BC in the single-factor analysis showed that early age at menarche,early menarche and long-interval duration of the first living births to the menarche are the risk factors of BC for women in Shenyang area,especially over the age of 35 premenopausal women.
