IntroductionTransient ischemic attack(TIA) is a common type of ischemic cerebrovascular disease, it is not only a common illness, but also a dangerous signal of cerebral vascular disease. It is the aura and the important preventable risk factor of complete stroke. The fifth international stroke congress report that transient ischemic attack account for more than 40% of the ischemic stroke in total and 10% of the overall stroke. The study result of Harvard university disclose that 50% of the ischemic stroke patient have the history of transient ischemic attack. The effective prevention and therapy of TIA will decrease the incidence rate and mutilation rate greatly. Therefore, it will of be of great clinical significance in preventing the transformation from TIA to cerebral infarction to intensify the study of the risk factor of high-risk TIA, to identify the risk factor for the transformation from TIA to cerebral infarction, and to make a individualized therapeutic regimen.ObjectiveTo discuss the relationship between the bad prognosis(developing to the cerebral infarction) and the last time of TIA, the frequency of attack, the serum triglyeride(TG), the total cholesterol(Tch), the high density lipoprotein(HDL), the low density lipoprotein(LDL) and the plasma fibrinogen(FIB).Methods1. We collect 110 patients with TIA from the out-patient and in-patient of department of neurology of the first affiliated hospital of china medical university from Nov. 2006 to Dec.2007. All the selected cases are consistent with the diagnostic criteria of TIA established in the 4th national cerebrovascular conference in 1995. We follow up the clinical data of patient in the first attack, one month after attack, three month after attack. We record the time of first TIA attack, the last time of each attack, the frequency of attack, serum TG, Tch, HDL, LDL, and plasma FIB in detail. The terminal event is cerebral infarction and death.2. According to the clinical follow up data of the 110 TIA patients in the first attack, one month after attack, three month after attack. We divide the 110patients into two group: the cerebral infarction-occuring group(CIO) and the non-cerebral infarction-occuring group(NCIO). We perform statistics analysis on the clinical data of the to group including the last time of TIA, the the frequency of attack, serum TG , Tch, HDL, LDL. and plasma FIB. All measurement data use mean±standard deviation((x|-)±s ), t-test is used for comparison, P<0.05 is the standard of differences with statistics significance. We deal with all the data with the SPSS14.0 statistics analysis software.Results1. 31cases(28.2%) of the 110 TIA patient became TIA within 3 months after the first attack of TIA.2. In the CIO group the last time of each TIA is 48.1±31.6min. The frequency of attack is 4.1±2.7time per day. The serum TG is2.04±1.49mmol/L.Tch is5.36±0.9 7mmol/L .HDL is 1.19±0.35 mmol/L,LDL is3.29±0.82mmol/L,FIB is 3.92±0.97g/L.3. In the NCIO group the last time of each TIA is 12.4±6.7min. The frequency of attack is 1.5±1.1 time per day. The serum TG is1.46±0.63mmol/L.Tch is5.18±0.92 mmol/L.HDL is 1.27±0.33 mmol/L,LDL is2.92±0.67 mmol/L,FIB is 3.44±0.77g/L.4. The Statistics analysis show that the comparison CIO group with NCIO group, the two group have significant differences in the last time of TIA (P<0.01), the frequen -cy of attack( P<0.05), the serum TG LDL FIB( P<0.05). while differences of the two group in serum Tch and HDL(P>0.05) have no statistical significance. Conclusions1. The longer the last time of attack, and higher of the frequency of attack of the TIA, the higher the risk to become cerebral infarction. The long last time of attack, and high frequency of attack of the TIA are the risk factors for cerebal infarction resulting from transient ischemic attack.2. The risk to become cerebral infarction is high in TIA patient whose serum TG LDL FIB is high, thus the high serum TG LDL FIB are also the risk factors for cerebal infarction resulting from transient ischemic attack.
