ObjectiveTo study the relation and clinical meaning between TLR4 and H.pylori by testing TLR4 expression and H.pylori infection in gastric mucosa with intestinal metaplasia, dysplasia and cancer. We can present a new thread of thought for testing and treating gastic cancer and precaneerous lesions.MethodImmunohistochemical technique was used to detect the expression of TLR4 in 120 cases with normal, intestinal metaplasia, dysplasia and gastic cancer mucosa. 14C -UBT and rapid uaease test was used to examed H.pylori infection.ResultThe expression of TLR4 in gastric mucosa with intestinal metaplasia, dysplasia and cancer was 90.0%,86.7% and 63.3%,respectively.And they were statistical difference as compared with that in normal mucosa (26.7%)( p<0.05).The positive rate of TLR4 expression in intestinal metaplasia and dysplasia was statistical difference as compared with that in gastric cancer mucosa ( p<0.05). The positive rate of H.pylori infection in gastric mucosa with intestinal metaplasia, dysplasia and cancer was 93.3%,90.0% and 66.6%. And they were statistical difference as compared with that in normal mucosa (33.3%)( p<0.05). The positive rate of H.pylori infection in intestinal metaplasia and dysplasia was statistical difference as compared with that in gastric cancer mucosa (p<0.05).The expression of TLR4 was 83.53% and 5.71%,respectively, where gastric mucosa were or not infected by H.pylori. Conclusion1.TLR4 expression in gastric mucosa with intestinal metaplasia, dysplasia is significantly higher than that in gastric cancer mucosa.2.In gastric mucosa with intestinal metaplasia, dysplasia and cancer ,it is proved that the infection of H.pylor have relations to the expression of TLR4.3. In gastric mucosa infected by H.pylori,LPS transduction pathway plays an important role in the precancerous lesions.
