Object: To find the existent problems and weaknesses of the prevention and healthcare work in the township hospitals in the poverty areas and evaluate the factors that affect the service ability of the prevention and healthcare of the township hospitals and establish the basis of the qualified and excellent prevention and healthcare service of the townshiphospitals in the poverty areas.Methods: Survey the characters, manpower, finance, capital construction and main equipment resources of the township hospitals in the sampling counties by the field research and the questionnaires; the working instances of the township hospitals in the sampling counties; the instances of the prevention and healthcare of the township hospitals in the sampling counties; analyze and evaluate the factors that affect the service ability of the prevention and healthcare of the township hospitals; bring forward the suggestions and countermeasures to advance the service ability of prevention and healthcare of the prevention and healthcare of the township hospitals in the poverty areas.Results:1.The payout makeup of the health career in the four sampling counties is not reasonable, the prevention and healthcare work has not been paid enough attention and this problem is still outstanding.2.The number of the working staff of the township hospitals per thousand persons and beds per thousand agricultural population are both below the average level of the whole province and nation in the year of 2003. 3. The sampling township hospitals do a good job at vaccination, newly delivery, feed guidance, health education propaganda, epidemic supervision and enfant check-up. The general check-up of the gynopathy diseases should be reinforced.4. The township hospitals undertake the prevention and healthcare items: feeding guidance93.2%, newly delivery 100.0%, vaccination 100.0%, health education propaganda 91.8% and the epidemic situation supervision 93.2%. The other prevention and healthcare service items are undertaken below 50.0%. The whole situation is not satisfying.5. Logistic regression analysis is used to analyze the four prevention and healthcare items carried out by the sampling township hospitals. These items are carried out for nearly 50 percent. The prevention and healthcare income has great significance that indicates that the strength of the staff's consciousness of prevention and the leaders'recognition can manoeuvre the activity of the township hospitals.6.The sampling township hospitals reported the primary public health incidents, the total number is 3,042 and the death number is 29. Among them the first five diseases are diarrhea, hepatitis, typhoid and food poisoning.Suggestions:1.Definite the responsibilities of the governments of each level in the public health and reinforce the organization and leadership of the public health;2.Put the double management system of the government and the health service department into practice, set up relatively absolute prevention and healthcare organization. The health service department takes charge of the health layout, operation guidance, sanitation supervision, personnel arrangement and the control of the prevention and healthcare expense. 3.The township hospitals must change the traditional service mode if they want to occupy the community medicine market, which is the necessary trend of the reform of the rural health sections and adapt to the need of the peasants of the new epoch.4.The most outstanding problem of the Guizhou public health is the control of the endemics and the infectious diseases, therefore the primary content of the prevention and healthcare is the solution of the most outstanding problem.5.The township hospitals should build rational human resource training, using and management system to stabilize the working staff and raise the level. It's suggested that the medical staff below 50 years old should be sent to be trained during 3 to 5 years. The medical colleges and the county hospitals can take the training task.6.The township hospitals should constitute the rural area health program according to the factors of the population, traffic, economy and service radius to favor the reasonable collocation of the sanitation resource.
