Analyses Of Status Quo For Out-patient Department In Harbin Children's Hospital And Investigation Of Construction For Out-patient Department

Harbin Children's Hospital was the largest comprehensive hospital for children in Heilongjiang province. In order to meet the new requirement for medical service in new century, Harbin Children's Hospital had raised twenty millions Chinese yuan for new comprehensive out-patient service. The new out-patient service will be completed very soon following the help and concern of government. How to construct a new serial management is going to be on the agenda. However the outdated mode of management had already conflicted with medical requirement and prevent our hospital from developing continuously. It is necessary for us to investigate a new style of hospital management which should contain the modern humane concept to substitute outdated one.ObjectiveRetrospectively investigate medical works and administrative situation of our hospital in 2006; analyze the problem in the style of management and status quo; approach a new way to build out-patient service and improve the level of management.MethodsDescriptive statistical method was used to analyze the dairy of out-patient service from January 2006 to December 2006 and the datum of statistical department over the years.Material1.We divided the dairy of out-patient service from January 2006 to December 2006 into many groups according to the kind of illness and clinical department. 2.We investigated the installation of clinical department in out-patient service and evaluated the doctors'workload.3.We consulted the statistical yearbook of health institution in Harbin and collected correlative datum.4.We consulted and analyzed the datum of many department in Harbin Children's Hospital, such as statistical department, medical department and so on.Results1.The medical staff in Harbin Children's Hospital were 615,which could hold 450 patients. There were 19 clinical department in out-patient service. The mean person-time of out-call in out-patient service was 29 in 2006.The patients in out-patient service in whole year were 437862.Every doctor in out-patient service medially treated 41.37 patients everyday.2.The department of out-patient service in Harbin Children's Hospital were composed of three parts: administrative department, clinical department and adjunctive department. The administrative department was medical management department. The clinical department involved 19 secondary department; Medical department, surgery and the department of five sensory organs were main parts. Medical department were divided into 8 departments. The department of five sensory organs were divided into three departments: department of stomatology, ear nose throat department and department of ophthalmology. There were department of adolescent psychological consult, department of traditional chinese medicine and many other correlative departments. The adjunctive department were divided into five parts: clinical docimastic department, physical examinational department, intravenous injection department, dispensary and financial department.3. The kinds of illness diagnosed in clinical department of out-patient service were more than 200 kinds in 2006. According to the international principal of illness classification and requirement of diagnosis in our hospital, we divided those illnesses into 11 kind of illnesses.4. According to the principal of illness classification described above in the study, it was showed: the number of patients with respiratory illness was in the first place, adolescent psychological illness was second. The sequence of other illnesses were: illness of five sensory organs, disease of digestive system, dermatic illness, infective illness, optic illness, surgical illness, nervous illness, hematological illness and congenital illness.5. The datum of our hospital over the years showed that the number of patients in out-patient service had increased from 9000 in 1955 to 437862 in 2006, the person-time had increased from 24.66 to 1199.62. The workload of out-patient service had increased year by year, fellowing the development of requirement in medical service.6. We could find the seasonal change of some illness according to the change of patients'quantity in out-patient service.7. The dairy of out-patient service should be written by standard.The correlative department should amend the errors and flaws of management about the datum of out-patient service. 8. We should allocate the medical resources reasonably in order to relieve the work pression in out-patient servic according to the characters of many departments and the quantitative change of patients in out-patient service.9.We should enforce humane management and increase children institution according to the character of Harbin Children's Hospital.Conclusion1. Harbin Children's Hospital was the largest comprehensive hospital for children in Heilongjiang province. There were 19 clinical department in out-patient service. The patients in out-patient service were 437862 in 2006 and increased 110167 compared with those in 2005(327695 person-time).The average person-time in out-patient were1199.62 in 2006.The illnesses in out-patient service were divided into 11 kinds and the number of patients with respiratory illness was in the first place(151301 cases), amounted to 56.19%.2. The disposition of clinical department in out-patient service were not standard.There were latent danger of infection and misdiagnosis.Our out-patient service was short of children character.The medical resources were shortage.The workload became more large.There were the errors and flaws of management about the datum of out-patient service.3."The patient first"should be embodied in the building of out-patient service.We should enforce the humane standard management. We should allocate the medical resources reasonably according to the quantitative change of patients in out-patient service, in order to relieve the work pression in out-patient servic. We should enforce monitoring and management about the dairy of out-patient service. The style of out-patient service must meet the character of Harbin Children's Hospital. In the initial stage of construction, we should combine new ideas of management with construction of out-patient service.
